re tender for supply installation and commissioning of 128 slice ct scan machine for department of radio diagnosis of scbmch cuttack , ct scan 128 slice with all accessories with specified quantity as asked in technical specification 1. the bidders have to quote the unit price of the base equipment with all accessories as asked in the technical specification including 5 ( five ) years onsite warranty, cost of installation & commissioning and all other incidental charges excluding gst against column-3 of excel boq file. 2. the bidders shall have to quote ( upload ) the breakup of prices of base equipment & accessories as asked in the technical specification in the price boqs ( excel files ) as well as in a separate pdf file ( format-b ) attached in the e-tender portal against this tender reference. in that case, the total unit price ( including 5 ( five ) years onsite warranty, cost of installation & commissioning and all other incidental charges excluding gst ) mentioned in the pdf file must be same as that quoted in column 3 of the price boq in excel file 3.in addition, the bidders have to quote the cost of any other accessories / disposable / consumable other than as asked in the technical specification with other spare parts required for functioning of all the quoted items in the separate price schedule format for each item attached as pdf file ( format-c ) in the e-tender portal. however, this shall not be taken into account for evaluation.
