The bidders have to enter a single rate for all the health institutions bidder is participating, the bid(s) of the bidder(s) entering multiple/different positive discount rate (in percentage terms) in BoQ shall be liable to be rejected. 3. No rates are to be filled in this annexure (physical), the rates are to be quoted online in uploaded BoQ. In case the rates are quoted in physical document (BoQ), the bid is liable to be rejected. 4. A bidder should bid for a minimum of 25 institutions (all 10 institutions of category A and minimum 15 institutions from category B), failing which its bid is liable to be rejected.5. The bidder offering highest positive discount rate (percentage) against the above provided rates shall be determined as L-1. Prospective bidders should note that the discounted rate (percentage) should be inclusive of the cost of any materials i.e. films/contrast/syringes etc. required for the respective CT-Scan service. The discounted rates shall also be inclusive of reporting of the scan by a qualified radiologist as prescribed in the RFP document.6. It is emphasised here that every bidder should quote discount rates keeping in view costs of various inputs required for providing the respective scan service. The discount rates quoted by bidder should factor in any kind of medical/non-medical item/input required for providing the said service(s). No additional cost, whatsoever, shall be payable to the service provider over and above the discounted price (arrived at on basis of discount rate quoted by the bidder) as per the price bid.Signature/Stamp of Authorized Signatory |
