Vendor to deploy Supervisor, Drivers, and Semiskilled Operators. Personnel is accountable for seamless service delivery. · Working Hours – 10 Hours in a day (07:00 to 17:00 Hours and 13:00 to 23:00 hours) · Manpower – Supervisor -3 ( 2 manpower- in day shift <(>&<)> 1 manpower in second shift), Labor- 22 ( 15 in day <(>&<)> 7 in second shift) · The contractor was tasked with maintaining toilets and fittings. Ensuring operational conditions throughout the service tenure. All the toilets installed in our premises should be cleaned every 3 hours. This is applicable during the night as well. · The contractor should keep a log record of cleaning and maintenance activities done which should be certified by the authorized AMNS person daily. AMNS personnel to validate and certify daily records. · Vendor to guarantee uninterrupted water availability in all toilet facilities. The contractor is responsible for maintaining adequate cleaning supplies and fittings inventory. · Contractor to perform timely and thorough cleaning of all tanks. The operations of cleaning toilets should be carried out in all shifts. · Vendor accountable for proper disposal of sludge/mud at designated locations. · The Permit to Work (PTW) system must be followed by the Contractors personnel where required within AMNS premises. · All minor breakages and maintenance issues are to be attended to by the vendor immediately. · Contractor shall ensure all his staff are aware of Company safety and fire regulations whilst on Company premises. · The Contractor shall ensure that the Personnel should be wearing a neat and clean uniform every day. The Contractor shall provide identity cards to its Personnel, duly authorized by the Contractor which would be subject to verification by the Company from time to time. The Company reserves the right to prohibit entry or removal of any Personnel without identity card or not dressed properly to the Premises. Verification of personnel through Company-approved identity cards. · The Contractor shall engage supervisor(s) to supervise the works of the Personnel engaged by the Contractor at the Premises. Any complaint regarding the Services shall be directed to the supervisors and the supervisors shall have to take immediate steps to resolve such issues and shall ensure that the Services are provided uninterruptedly throughout the validity period. The Contractor shall have full supervision and control over its Personnel engaged in providing the Services within the Premises to ensure that the Personnel shall abide by all the terms and conditions of AMNS and also those terms and conditions that may be stipulated by the Company from time to time for providing these Services by the Contractor. The Contractor shall be solely responsible for the acts and omissions of the Personnel and its supervisor. Recruitment of personnel aged between 18 to 45 years for service execution. · Strict prohibition of cycle/two-wheeler movement within Plant Premises. · The contractor is responsible for providing safe and efficient transportation for personnel within the premises. · Vendor must ensure availability of sufficient buses/vehicles to ply their associates inside plant premises. Vendors to ensure safe transportation of their associates from their place of residence to respective work location in the plant and back. The movement of persons inside the plant by bicycle/two wheelers is prohibited. The contractor shall make replacement of persons at no additional cost to AMNS. · The Contractor shall perform the Services as per the instructions of the authorized representative of the Company, which may be given daily or otherwise. The authorized representative of the Company shall monitor/check the Services are being provided by the Contractor. Any problem / mistake found / observed shall needs to be corrected / resolved by the Contractor promptly. · Performance of the contractor shall be monitored continuously to ensure they meet acceptable standards of service. If AMNS is not satisfied with the contractors work the contract shall be terminated, at no additional cost to AMNS. · Contractor shall be deemed to have full working knowledge and have obtained all necessary information and data of Company Facilities at which Services are to be performed, including, where applicable, prevailing location, physical conditions, hygienic state, access, contingencies and all other circumstances under which the Services shall be performed. · Contractor vehicles entering the AMNS site shall be well maintained and should be as per AMNS Safety Standards and shall be inspected by Company designated representative. · The co ntractor shall ensure that his workforce is covered under the required insurance as per government rules and regulations. · The contractors shall prepare the security gate passes for their personnel and furnish all information requested bypass the issuing authority. · The cost of repairs arising during the operation should be borne by the contractor. Necessary manpower, fuel, lubricants, tools <(>&<)> tackles, equipment, and spare parts shall be made available by the Contractor as a normal scope to attend the breakdowns. · The contractor shall maintain a logbook in duplicate giving full operation details, preventive maintenance, and Breakdown records, and obtain the counter signature of the Employer daily. Original log sheets shall be submitted to Employer at regular intervals as directed by Employer and before submission of monthly bills. Supplier to submit signed hard copy of load chart of all the offered Equipment’s and confirm that the equipment’s will function as per the load chart. The logbook shall be certified by Employer representatives/line managers on a daily basis. The supplier also has to maintain the maintenance logbook of the Equipment’s and submit it to EMPLOYER on a regular basis. A supplier must submit a copy of RC book and insurance certificate, fitness certificate, DL of the operator and other related documents to Employer during Contract Period · Accommodation <(>&<)> transport of operator and mechanic (if required during maintenance or breakdown attention) to be arranged by the service provider. · Any expense of the operator/ mechanic to be borne by the service provider. Responsibility for Equipment or any loose items of equipment will be taken care of by the service provider. · All required statutory arrangements for a gate pass to be arranged by the service provider. · All statutory safety c hecks must be ensured by the service provider all the time. Penalties Overview: The proposed penalty system consists of the following penalties: · Absenteeism and Late Attendance: To foster punctuality and attendance adherence, a penalty of Rs 2000 per instance will be implemented for unexcused absences and instances of late attendance without prior notice. · Substandard Housekeeping: Upholding workspace hygiene and order is vital; therefore, a penalty of Rs2000 will be enforced for inadequate housekeeping practices. · Quality of Wor k: Ensuring the quality of output is imperative; hence, a penalty of Rs2000 per instance will be enacted for work that falls below established standards. · Maintenance <(>&<)> Cleaning Material (which includes bacteria, and toilet cleaning material) on the actuals list to be submitted along with the invoice bill if any discrepancy it will be penalized as per the Admin user. It’s important to keep the Admin team informed ahead of time about any repair and maintenance activities, as well as the supply of consumables. · Job not done as per specifications/instructions # In case manpower does not perform the job as per directives/ checklist given or the quality of work is not good. In such case 300/per day/ per Instance will be levied on the vendor. · Delay arrival of manpower on duty# Reporting time on Duty is 7 am/1 pm, in case of manpower report late on duty in such instance Rs.300/per day and per instance will be levied on the vendor. · Manpower without Uniform and PPEs # If manpower was found without PPEs and Uniform then Rs.300/Day/Instance will be levied on vendors. · In case of short fall in agreed deployment, the invoice will be processed as per proportionate deployment. Payment shall be made as per actual deployment and minimum wage basis. In case absenteeism of manpower is more than 5% of total man days. In such condition Rs.300/per day per person will be levied on manpower on proportionate billing. · If performance remains unsatisfactory for a period exceeding three months, a warning notice will be issued. Should there be no improvement following the warning, the contract will be terminated. 2.Theft Cases: Instances of theft undermine our trust and integrity; therefore, a penalty of Rs5000 will be incurred in such cases. Note: Upon recieving of the Purchase Order, services shall be delivered at Upstream Project site within a timeline of seven business days.
