(b) In OPD slip there must be an options to print customized logo and messagesfor various Govt. programs and schemes. 2 INDOOR PATIENT REGISTRATION with (1) IPD Ticket Printed by laser / inkjet printers with patientsName, Age, Sex, Caste, Father/Husbands name, Address, City, Mobile number, IPD Ward, Provisional diagnosis, Scheme, Scheme Id , Referred from, and Doctors name printed in the details. 1 Page A-4 size 70 GSM Paper 24X365 IPD Registration 3 PATIENT SERVICE RECEIPT (a) Department wise Receipt of various services provided by RogiKalyanSamiti e.g. Pathology, X-Ray, Sonography, Blood Bank, ICU, Special wards, Ambulance etc. (b) Receipt must be Printed by laser / inkjet / thermal printer with full information of patient collected during registration and details of Services to be provided, quantity and specification of service, name of Department. (c) If the APL patient is charged for any services (RKS Sewashulk), the charges of services must be printed on receipt in figures in words. (d) Each service receipt must have its unique Bill no., date and time of bill creation, name of Computer Operator,andPatients Unique registration number must be printed on each and every receipt in bold characters. 1 Page A-4 size 70 GSM Paper 24X365 OPD Registration counter 4 DIAGNOSIS TEST REPORTING (a) Computerized Pathology test reporting, X-Ray, USG diagnosis and comments reporting on paper and distribution to patients. 1 – 3 Pages A-4 size 70 GSM Paper 24X365 OPD Registration counter 5 PATIENT DISCHARGE TICKET a) Discharge ticket, Referral slip, to be issued for each patient.
