1. The Subject Of The Contract Is The Provision Of Assistant Services As Part Of The Project Home Teleopieki Program-Senior Policy Support Program And People With Disabilities For 1 Person With A Significant Or Moderate Disability At The Age Of 18. Disability Co-Financed By The European Social Fund+ Under European Funds For Podlasie For 2021-2027. -Assistant Service Includes Supporting People With Disabilities In Performing Basic Everyday Activities, Necessary For Active Social, Professional And Educational Functioning Of People With Disabilities, In Particular: - To The Doctor, To Service Points And Other Public Places And Assist In These Places;-Spending Free Time, Including Support During Cultural, Sports And Recreational Activities, E.G. Signage To Sign Language;-Support In Movement;-Support During Educational Classes And In The Learning Process; -Itanicity Facilitating Communication With The Environment In The Case Of Deaf People, Or Helping The Disabled In Physical Activities Impossible To Perform Independently At The Workplace Or In Search Of Work.-Assistant Services Being The Subject Of The Contract Will Be Performed On Business Days, As Well As On Saturdays, Sundays And Holidays From 6.00 To 22.00. -1 Hour Of The Assistant Service Is Assumed To Be 60 Minutes. The Mentee.-Road Of Services And The Hourly Scope Will Depend On The Personal Situation Of The Person Using The Service. The Detailed Scope And Dimension Of The Provided Services Will Be Accepted By The Participants Of The Project And The Person Providing Assistant Services-The Number Of Hours Planned As Part Of The Service: 20 Hours Per Month Per 1 Person, Which Gives 280 Hours In Total (1Os * 20H * 14 Months)-The Landlord Reserves That The Given Number Of Hours Is The Maximum Amount, Which May Not Be Reduced Due To What The Contractor Will Not Have The Right To Any Claims Of This Title. On The Work Card, The Mentee Confirms The Performance Of Services With His Own Signature. The Work Card May Be Confirmed By A Family Member In The Event Of An Incapacity For A Signature By The Mentee Due To His Health;-Information About The Temporary Unaware Of The Services;-The Ordering Of The Ordering Party After A Month, No Later Than The 5Th Day Of The Following Month Of The Specification Containing The List Of The Number Of Services Completed In The Previous Calendar Month For Individual Charges.
