1) The Subject Of The Contract Is The Provision Of Care Services (Referred To In Article 50 (3) Of The Act Of 12 March 2004 On Social Assistance) For 15 People, Clients Of The Social Welfare Center In Nowa Karczma In The Nowa Karczma Commune, In The Amount Of 3.480 Hours In Total, I.E. On Average 145 Hours In A Month (On Average 232 H/Person) In The Period From September 1, 2025 To December 15, 2027.2 Point 1, The Contractor Will Be Obliged To Provide In A Continuous And Appropriate Manner In Terms Of The Quality Of The Process Of Providing Services For 7 Days A Week, By Properly Determining With Persons Providing Care Services Of Hours And The Commissioned Dimension And Scope Of Services. 3) Service Time: 1 Hour Of Service Equals 1 Hour Of The Clock, I.E. 60 Minutes. 4) The Concept Of The Number Of Hours Of Services Provided Should Only Be Understood As The Actual Time Of Providing Services Without Preparatory Activities, I.E. Without The Time Needed For Access Or Access. 5) Number Of Hours 3.480 In Para. 1 Is An Estimated Number, The Dimension Of Services Rendered Is 145 Hours A Month For About 15 People. 6) The Number Of Hours May Change, Because It Cannot Be Determined Exactly Due To The Specificity Of The Order, Which Is Characterized By Variability Of Needs Due To The Changing Health And Life Situation Of People Requiring Help. The Remuneration Due To The Contractor Will Be Determined On The Basis Of Gross Price 1 Hour Of Care Services Given In The Offer. Quantitative Changes Of Hours, As Well As Reducing The Value Of The Contract, Do Not Require An Annex To The Contract. As Part Of The Offer Price, The Employer Reserves The Right Not To Use The Entire Hours Ordered. The Contractor Is Entitled To Remuneration For The Number Of Actually Rendered Services And Will Not Submit Any Claims As To The Implementation Of The Remaining Part Of The Contracting Authority. 8) The Subject Of The Contract Includes The Performance Of Care Services For People Who, Due To Age, Illness Or Other Reasons, Are Not Able To Satisfy Everyday Life Needs, Hygiene Care, Ordered By The Doctor And Provide Contacts With The Environment And Also For People Who Require The Help Of Other People, Who Require The Help Of Other People, And The Family Of Such Assistance Cannot Provide Particular Care, Ensuring Rational Use Of Technical Means And Employees And Care For Customer Property. The Contractor Is Obliged To Hire Non -Criminal, Physically And Intellectually Efficient, Capable Of Performing Physical Work, Having The Ability To Maintain Correct Interpersonal Contacts. 11) The Contractor Will Be Obliged To Perform The Subject Of The Contract In Accordance With This Subject Of The Contract, By The Act Of 12 March 2004 On Social Assistance (I.E., Journal Of Laws Of 2024, Item 1283, As Amended).
