Tenders Are Invited For Provision Of Services For The Implementation Of Specialized Classes As Part Of The Wokro Task 2.4 Tasks Of The Program:

Part 1 - Speech Therapy Classes Package I - In An Estimated Amount Of Up To 112 Hours; 1. The Subject Of The Contract Is The Implementation Of Services In The Field Of Early Development Support For Children Under The Comprehensive Support Program For Families Behind Life In 2024, Constituting An Attachment To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 On The Program Of Comprehensive Support For Families After Life (M.P. 2022 Item 64) Hereinafter Referred To As The Program .2. Services Constituting The Subject Of This Contract Will Be Provided In The Form Of Individual Classes With Children From Birth To Their School Education. 3. A Full Clock Hour (60 Min.) 4 Is Assumed For One Hour Of Providing Services. The Employer Points Out That The Concept Of The Service Of The Service Of Services Should Be Understood Only By The Actual Time Of Providing Services For The Child Or For Him (This Time Does Not Include, For Example, The Time Necessary To Move The Person Providing Services From One Child To Another, If The Person Provides Services In Several Children In One Day). 6. The Costs Of Travel In The Scope Of The Implementation Of The Subject Of The Contract Are Covered By The Contractor Under The Remuneration Specified In The Offer Constituting The Maximum Commitment Of The Employer. 7. The Employer Shall Specify The Detailed Time And Place Of Provision Of Services In The Schedule Of Classes Determined Taking Into Account The Needs Of Participants Of Individual Therapies In Consultation With The Contractor. 8. Classes Will Be Implemented At The Team Of Educational And Educational Institutions In Gołdap, Address: Ul. Wojska Polskiego 18, 19-500 Gołdap. 9. Classes Will Be Implemented In: A Therapeutic Room Equipped With Equipment And Aids Necessary To Conduct Classes Being The Subject Of This Contract. 10. Classes Will Take Place From Monday To Friday Between 1500 And 1800 And On Saturdays Between 800 And 1600.11. Classes Will Take Place In Accordance With The Number Of Hours Granted To The Child, Determined By The Employer According To The Agreed Schedule. 12. The Service Provided By Each Of The Persons Addressed To The Implementation Of This Order - Hereinafter Referred To As Specialists, Will Be Conducted Depending On The Childs Needs Up To 5 Hours A Week, In Accordance With § 1 Para. 4 Of The Regulation Of The Ministry Of National Education Of September 5, 2017 On Detailed Tasks Of Leading Coordination, Rehabilitation And Care Centers (Journal Of Laws 2017 Item 1712, As Amended). 13. The Scope Of Specialists Duties Includes: 1) Providing Parents With Specialist Information On The Childs Developmental Problems; 2) Ensuring Teaching And Therapeutic Assistance Necessary For The Implementation Of The Task; 3) After Establishing A Team Of Specialists For A Specific Child, Establishing Directions And A Schedule Of Activities Undertaken In The Field Of Early Support And Support Of The Family; 4) Establishing Cooperation With The Ins
