The Subject Of The Contract Is The Provision Of Services In The Field Of Veterinary Care Over Homeless Animals And Free Living Cats In The Ursynów District Of The Capital City Of Warsaw. General Assumptions Of The Contract1) The Contractor Must Accept Animals And Perform Services Covered By The Order Of The Education Within The Meaning Of The Act Of 18 December 2003 On Medicinal Plant For Animal (I.E., Journal Of Laws Of 2019, Item 24). For This Purpose, Experience And Ability To Deal With Free -Living And Homeless Animals Are Necessary, In Particular Predicting The Reactions Of Animals Found In Stress Or Aggressive. 3) Free -Living Animals (Wild), In Accordance With Art. 4 Points 21 Of The Act Of August 21, 1997 On Animal Protection (I.E., Journal Of Laws Of 2022, Item 572), These Are Unsuccessful Animals Living In Conditions Independent Of Human. For Art. 21 Above Of The Act, Free -Living Animals Are A Nationwide Good And Should Have Conditions For Development And Free Existence. 4) Social Guardians Of Free -Living Cats (Hereinafter Referred To As Carers), They Are Persons Included In The Database Of Social Guardians Of Free Living Cats, Conducted By The Department Of Environmental Protection For The Ursynów District Of The Capital City Of Warsaw (Hereinafter Woś), Who Voluntarily And Freely Care For Free -Living Cats By Feeding And Taking Care Of Their Welfare. 5) Carers Have The Right To Full Information About The State Of Animal Health And About Treatments Performed By The Medicine For Animal (Hereinafter: Zlz). Ii. The Scope Of Ordering Includes The Performance Of Veterinary Services Listed In The Price Form (Annex 2 To The Contract). The Expected Amounts Of Individual Treatments Provided In The Attachment During The Period Of The Contract Are Approximate Amounts And May Change (Increase Or Reduce) During The Performance Of The Contract - According To The Actual Needs During The Period Of The Contract, With The Total Maximum Value Of All Actually Actually Made Veterinary Services Will Not Exceed The Total Offer Price Of The Selected Contractor With Whom The Contract Will Be Signed. The Subject Of The Contract Is Determined By: A) Description Of The Subject Of The Contract - Annex 3 To Swz, Which Is Also Annex 1 To The Planned Provisions Of The Contract; B) The Proposed Provisions Of The Contract - Annex 2 To Swz; C) Price Form - Annex 4 To Swz, Which Also Constitutes Annex 2 To The Planned Provisions Of The Contract;
