The Provision Of Clinical Massage Services In Accordance With The Individual Demand And Health Of Seniors - Project Participants - Dependent People, Affected By Illness Or Disability, Having Health Problems, Requiring Support In The Field Of Recovery, Condition And Improvement In Physical Fitness: - The Service Will Be Provided For A Maximum Of 120 People - Project Participants After 5 Treatments, The Total Performance Of 600 Medical Services Is Planned Clinical. The Order Will Be Carried Out In The Period From The Date Of Signing The Contract To 31.12.2026, Of Which In 2025 It Is Planned To Provide Medical Clinical Massage Services For 60 People Each 5 Treatments And In 2026 The Provision Of Medical Clinical Massage Services For 60 People Each 5. In A Unique Situation, At The Contractors Request, The Employer May Agree To Change The Number Of Treatments In A Given Year - 600 Services Should Be Provided By The End Of 2026. The Duration Of One Medicinal Massage Treatment Is A Minimum Of 60 Minutes. The Detailed Number Of Services In A Given Month Will Depend On The State Of Health And The Needs Of Patients /Patients Covered By The Project;- Lack Of Imposed Fixed Days And Hours, The Services Will Take Place On Monday-Sobota Between 08:00 (The Earliest Hour Of Service Start)- 21:00 (The Latest Time Of The Service At The Latest). The Contractor Will Be Obliged To Determine Together With The Ordering Party Of A Detailed Service Schedule Containing Specific Dates And Hours Of Service Provision. - Support Services Of A Masseur / Physiotherapist Technician Will Be Provided At The Patients Home Located In The Bieliny Commune. - The Price For The Service Also Includes Traveling Costs To The Patient;- The Contractor Will Provide Appropriate Equipment And Materials Necessary For The Proper Provision Of A Massage Table, Massage Instruments, Etc. Massages Will Be Performed Only On The Massage Table. The Contracting Authority Will Impose A Financial Penalty On The Contractor For The Lack Of The Above-Mentioned Equipment;- The Contractor Is Obliged To Guarantee Personal Protective Equipment, I.E. Disinfectant Fluid, Mask And Disposable Gloves For Persons Providing The Service;- The Contractor Must Be Available;- Payment For The Service Rendered Will Take Place On The Basis Of A Monthly Service Card (Annex 1 To The Contract) Containing Information On The Number Of Services Provided) (Which Is The Basis For Issuing An Invoice Or Vat Invoice By The Contractor) Signed By Both Parties- A Contractor And Contracting Authority- At The End Of Each Calendar Month In Which The Service Will Be Provided;- The Contractor Will Be Obliged To Constantly Cooperate And Consult With The Contracting Authority At Every Stage Of The Contract At Every Stage Of The Contract;- The Contracting Authority Reserves The Right To Control The Implementation Of The Subject Of The Contract At Every Stage Of The Work.
