Jyawardene Centre, No. 191, Dharmapala Mawatha, Colombo 07 on payment of the non-refundable fee of Rupees 1,000.00 to the Shroff Section of the Ministry at No.09, Philip Gunawardene Mawatha, Colombo 07 (should be registered separately for each field) on working days of the week from 09.00 a.m. to 3.00 p.m. during office hours and send same addressed to Secretary, Ministry of Sports and Youth Affairs, Procurement Section, 3rd Floor, J.R.
