1. The Subject Of The Contract Is The Provision Of A Family Assistant Service That Supports A Maximum Of 13 Families Living In The City Of Chełmno And Participating In The Project. 2. For The Direct Performance Of The Service Constituting The Subject Of The Contract, The Contractor Is Obliged To Direct At Least Two People - Family Assistants Who Meet The Requirements Set Out In Art. 12 Of The Act Of June 9, 2011 On Supporting The Family And Foster Care System (Journal Of Laws Of 2025, Item 49) And Additionally Have A Minimum Of 2-Year Experience In Working With The Target Group, I.E. With Families Affected By The Problem Of Inheritance Of Poverty And Social Exclusion. The Assessment Of Having By Persons Addressed To Directly Perform The Service Of The Requirements Referred To In The Previous Sentence Belongs To The Contractor. 3. One Family Assistant Can Work With A Maximum Of 8 Families. 4. The Tasks Of The Family Assistant Will Include Comprehensive, Individualized Family Support, In Accordance With Art. 15 Of The Act On The Support Of The Family And The Foster Care System, In Particular In The Field Of: Solving Basic Social Problems (Housing, Material, Health, Legal), Attention To Solving Psychological Problems (Emotional, Family, Problems In A Peer Group), Supporting Families In Social Activity, Solving Conflicts With Neighbors, Family Members Or Employees Of The Institution. The Assistants Tasks Will Also Include Motivating To Improve Professional Qualifications Or Continue Education, Help In Choosing The Right Vocational Courses Or School, Help In Breaking Complexes And Crises Resulting From Learning Failures. 5. The Service Constituting The Subject Of The Contract Will Be Implemented At The Place Of Residence Of A Given Family Or In The Place Indicated By It, On Days And Hours Agreed With The Family, Also In The Afternoon And On Days Off Work. 6. The Planned Total Dimension Of The Service During The Contract Processing (Duration Of The Project) Is 4800 Hours (Clock), While The Contracting Authority Requires The Provision Of A Service Of A Minimum Of 10 Hours A Month For Each Family With The Possibility Of Increasing The Number Of Hours According To Individual Needs Resulting From The Work Plan With The Family.
