This Tender Includes A Contract With The Region Of Southern Denmark On The Operation Of The Care Center Svalegangen, Which Is A Treatment Site Targeted Homeless, Including Among Other Things. Functionally Homeless, In The Region Of Southern Denmark. The Purpose Of The Treatment Site Is To Establish Care Places For Citizens In Homelessness After The End Of The Hospitalization. The Target Group Is Citizens Of Homelessness Who Need Simultaneous Health And Social Professional Support And Care For A Shorter Period Immediately After Admission To One Of The Hospitals (Including The Psychiatric Hospital) In The Region Of Southern Denmark. In This Way, The Care Center Contributes To Prevention And Health Promotion In Relation To The Individual Patient. The Purpose Of The Stay Is That The Homeless Person Experiences An Improved Physical Health In Relation To The Specific Illness And An Immediate Improved Mental Health, And That He Or She Gets Better Contact With Existing Relevant Offers. The Offer For The Homeless Consists Of A Stay Of Up To 14 Days, Where The Homeless Is Offered Care, Counseling And Psychosocial Attendance. The Lease Is Attached As Part Of The Tender Documents. The Upcoming Supplier Will Be Able To Take Over The Lease (Excluding Furniture), But This Is Not A Requirement.
