Tenders Are Invited For Delivery Of A Disabled Child With Medical Transport To The Iskierka Education And Tree Therapeutic Center In Grybów In The 2025/2026 School Year Together With Providing Care.
1. The Subject Of The Contract Is To Ensure The Delivery Of A Disabled Child By Medical Transport Along With Providing Care During Delivery To The Iskierka Educational And Tree Therapeutic Center, The Revalidation And Educational Center In Grybów In The 2025/2026.2 School Year. The Expected Number Of People Covered By Delivery In The 2025/2026 School Year Is 1.3. Medical Transport Should Be Implemented Using A Specialized Sanitary Transport, Meeting Technical And Qualitative Features, And Having Equipment Specified In Polish Standards Transferring European Standards Harmonized Regarding The Road Ambulance Suitable For Transporting A Person In A Lying Position. 4. The Driver Must Have The Right To Drive Privileged Vehicles Within The Meaning Of Art. 106 Of The Act Of 5 January 2011 On Vehicle Drivers (I.E., Journal Of Laws Of 2024, Item 1210, As Amended). 5. The Childs Guardian May Be A Nurse With Qualifications And Rights Required For Nurses Or A Paramedic. 6. The Driver Of The Vehicle Cannot Be A Guardian At The Same Time. 7. In Connection With The Childs Attacks In The Child, It May Be Necessary For The Guardian To Administer During The Transport Of A Relaxing Medicine Previously Transferred By The Childs Parent. 8. The Driver And Guardian Are Obliged To Bring And Take A Child Out Of The Vehicle. 9. The Delivery Will Be Carried Out Daily With The Exception Of Public Holidays And Holidays In The 2025/2026 School Year With A Vehicle Adapted To Transport Disabled Children. 10. The Delivery Will Be Carried Out In Accordance With The Timetable Developed Jointly With The Ordering Party And Adapted To The Childs Class Plan Using The Delivery. Later Changes In The Timetable Require The Acceptance Of The Employer. 11. The Contracting Authority Reserves The Right To Change The Scope Of The Order Resulting From Organizational Reasons. 12. The Order Includes The Childs Delivery To The Iskierka Education And Tree Therapeutic Center, The Revalidation And Educational Center In Grybów With The Childs Return Transport To The Place Of Residence. 13. The Service Contractor Is To Provide Punctual, Safe, Efficient And Trouble -Free Delivery. The Child Is To Be Brought To The Center Not Earlier Than 20 Minutes Before The Start Of Classes And No Later Than 10 Minutes Before The Start Of The Course. 14. In The Event Of A Failure Of The Designated Vehicle, The Contractor Will Provide A Replacement Vehicle That Meets The Appropriate Technical Requirements In Road Traffic. 15. In Winter, The Vehicle Transporting A Child Must Be Heated. 16. The Carrier Insures The Vehicle And Passengers From All Damage Caused During Transport And Remaining In Connection With Transport. 17. The Settlement Of The Delivery Will Be Made Monthly On The Basis Of A Correctly Issued Invoice/Account. 18. Order Performance Date 01.09.2025-31.08.2026 19.
