Tenders Are Invited For The Provision Of Veterinary Services For Dogs And Official Horses In The State Of The Provincial Police Headquarters In Łódź And Units Of The Łódź Garrison.

Part 1: Provision Of Veterinary Services For Business Dogs And Withdrawal Dogs From Service In The Police. 1. The Contractors Obligations Include In Particular: 1) Veterinary Treatment And Care For Service/Withdrawal Dogs; 2) Participation Of A Veterinarian In Periodic Dog Examinations At The Headquarters Of The Ordering Party, The Participation Of A Veterinarian In The Work Of The Team Dealing With The Organization Of The Use And Maintenance Of Dogs, In Accordance With Ordinance No. 91 Of The Chief Police Commander Of The Police Of 04.10.2022 - According To The Needs Of The Ordering Party. 2. The Detailed Scope Of Services Rendered Was Indicated In The Detailed Description Of The Subject Of The Contract-Annex No. 3.1-3.2. To Swz (According To The Part) To The Application And The Assortment And Price Form-Annex 2.1.-2.2. To Swz (Subsequently To The Part). 3. The Performance Of Services Covered By The Contract Will Take Place At The Headquarters Of The Contractor. If Necessary, The Services Will Be Performed At The Place Of The Dog In The Łódź Voivodeship (The Order Will Be Delivered By A Service Dog Guardian Or Informing By Phone In The Event Of A Service In The Łódź Voivodeship). 4. The Contractor Declares That He Has Appropriate Housing Conditions And Technical And Medical Equipment Necessary For The Proper Performance Of The Services In The Event Of The Need To Perform The Service At The Headquarters Of The Conference. 5. The Provision Of Services Will Take Place As Far As Possible Outside The Order, Subject To Paragraph 7 And 8 Contained In The Template Of The Contract Constituting Annex 5.1-5.2 To Swz (Subsequently To The Part). 6. If The Unplanned Service Is Necessary, The Contractor Shall Start Diagnosing The Dog Not Later Than Within 2 Hours. From Notification. 7. In The Case Of The Provision Of Planned Services - The Services Should Be Provided Within 5 Days (Offer Evaluation Criterion) From The Date Of Sending The Order. 8. The Contractor Will Perform Services At The Prices Contained In The Assortment And Price Form. Constituting Annex 2.1-2.2 To Swz (Subsequently To The Part). 9. Estimated Amounts Of Services Provided In The Assortment And Price Form Are Not Binding On The Ordering Party In The Performance Of The Contract, They Are The Basis For The Contractor To Draw Up The Offer. The Actual Number And Services Will Depend On The Needs Of The Ordering Party And His Budget Funds. The Person Undertakes To Provide Services At A Level Of Not Less Than 60% Of The Value Of The Contract Referred To In § 6 Para. 1 Of The Draft Contract. In The Case Of Commissioning Services Of A Lower Value Than Specified In § 6 Paragraph 1, But Not Less Than In The Preceding Sentence, The Contractor Will Not Serve Any Claims Against The Contracting Authority. 10. The Employer Reserves The Right To Change The Number Of Dogs, A Change In The Number Of Services Provided Specified In The Assortment And Price Form. Such A Change Will Not Change The Terms Of The C
