1. The Subject Of The Contract Are Construction Works - Constituting Task 1, As Part Of The Task Entitled Development Of The Emergency Medical System By Building A Landing Site For The Hospital Emergency Department Of The Provincial Hospital Of Zofia From Zamoyski Tarnowska In Tarnobrzeg. 2. A Detailed Description Of The Subject Of The Contract For Task 1, I.E. Construction Works, Including: 1) Execution Of A Label For Helicopters On Registration Plot No. 499/6, Located In Tarnobrzeg, 2-Kajmes. The Task Includes The Implementation Of Construction And Installation Works, Including: A) Making A Landing Plate And An Access Road, B) Performing Protection Against Bystanders By Making A Fence And Entrance Gate, C) Delivery Of The Technical Container Of The Landing Of The Landing Pad, D) Making A Labeling Of The Landing Plate, E) Execution Of The Power Supply System And Control Of Landing Installations, F G) Making The Navigation Lighting Of The Landing Pad Together With Power Networks, H) Installations Of The Light Track Indicator Of The Descent, I) Installation Of Ground Wind Direction Indicator, J) Installation Of The Control And Monitoring System, Enabling The Attachment Of Lighting Systems And Navigation Assistance From The Hospital Emergency Department And Preview Of The Landing Area, K) Installation Attaching Lighting Systems And Navigation Aid From The Helicopter Deck. 2) Developing The Necessary Formal Documentation And Obtaining No Later Than As At The Date Of Final Reception, All Necessary Decisions And Formal Permits Enabling The Operation And Use Of The Landing Pad (The Contracting Authority For The Representative Of The Contractor Of The Relevant Power Of Attorney), Including: A) Development Of The Landing Instructions And The Agreement In The Air Emergency Service And At The Polish Air Shipping Agency; B) Development Of The Landing Plan For The Landing Site And Agreement In The Air Ambulance; C) Obtaining A Decision Of The President Of The Civil Aviation Office On The Entry Of The Landing Pad To The List Of Landing Grounds Kept By The Civil Aviation Office; D) Obtaining A Positive Report Of The Landing Control By The Air Ambulance Service And In Consultation With The Employer To Organize And Coordinate The Training Of Technical And Medical Staff Of The Contracting Authority From The Service Of The Landing Site, 3) Technical Review At Least Once A Year Or In Accordance With The Terms Of The Manufacturers Warranty, 4) Guarantee Service On The Principles Indicated In § 10 Of The Contract, 5) Instruction Of Designated Ordering Employees In The Field Of Service And Maintenance Subject Of The Order. 6) The Subject Of The Contract Specifies: For Works: Design Documentation, Technical Specification Of The Execution And Acceptance Of Works, And In The Scope Of Organization Of Works - A Local Vision Of The Contractor, Which Will Take Into Account The Existing State And Related Works Of The Necessary Security And Correct Work Technology, And Transpo
