The Subject Of The Contract Is The Implementation Of The Sentence Entitled: Lease Of Defibrillating Vests Along With The Supply Of Patients For The Duration Of Treatment For The Needs Of Sp Zoz Csk Um In Łódź. 2. The Subject Of The Contract Consists Of The Following Options: 2.2.1. Service For A Three -Month Treatment Scheme For 20 Patients Or 2.2.2. Service For A One -Month Treatment Scheme For 60 Patients (Optionally In The Case Of Patients Requiring A Different Treatment Scheme) Minimal Components Of The Device:  Defibrillating Vest Monitor,  Belt With Ecg Electrodes And Defibrillating Electrodes,  2 Batteries,  2 Patient -Size Clothes,  Battery Charger Case For Defibrillator,  Bag For Accessories. 3. Cpv: 33190000-8 Various 35113430-2 Safety Vest, 33190000-8 Medical Devices And Medical Products 4. The Contractor Leases The Device-A Vest For The Treatment Of A Given Patient. The Amount Of Monthly Remuneration For The Device Must Include At Least: Patient Training And Matching The Device, Including Additional Training If Necessary, Activation And Adding A Patient To The Monitoring System, A 24 -Hour Hotline With Technical Support Available To The Patient In Polish, In The Event Of A Round -Time Failure, The Possibility Of Replacing The Equipment, As Well As Other Logistics Expenses Related To The Receipt And Delivery Of The Device. 5. The Contractor Will Be Notified Of The Need To Supply The Patient With A Vest. On The Day Of The Discharge, The Contractor Is Obliged To Provide The Device, Install It To The Patient, Train The Patient In The Use And Operation Of The Equipment, Sign A Lease Agreement With The Patient And Draw Up A Protocol Of Transferring Equipment To The Patient. The Patient Must Be Monitored By The Contractor Throughout The Use Of The Device, While The Results Of The Contractor Are Obliged To Report To The Employer, As Indicated By The Contracting Authority. 6. The Contractor Is Obliged To Collect The Device From The Employer (From The Hospital Ward / Clinic) After The Period Of Treatment, Which Is Informed By The Contracting Authority In Accordance With The Contract. 7. The Contractor Is Obliged To Issue An Invoice In Accordance With The Provisions Of The Contract. 8. The Order Includes 1 Package - The Ordering Party Does Not Allow Partial Offers. 1. The Contract Execution Date The Contract Will Be Carried Out In Accordance With The Current Demand Of The Ordering Party, In The Period From The Date Of Signing The Contract For A Period Of 12 Months On The Attitude Of Orders / Orders, With The Possibility Of Changing The Date Of The Contract, In Particular The Extension Of The Deadline. 2. The Deadline/Orders Required By The Ordering Party: 2.1. Order /Order Processing Deadline - Within Max. 48 Hours From The Date Of The Order / Order. 2.2.
