1. The Subject Of The Contract Is: The Provision Of Insurance Protection For Employees Of The Słowiński National Park As Part Of Group Health Insurance. The Service Includes Insurance In The Scope Of: 1) Medical Care For Employees Of The Ordering Party And Their Family Members In The Following Variants: A) Employee Variant - Which The Employee Of The Ordering Party Is Covered, B) A Partner Variant - Which The Employee Of The Contracting Authority And A Close Person Of The Employee Are Covered, With A Close Person Is Considered A Close Person - A Spouse, Cohabitation, Cohabitation, And Own And Adapted Children, C) Family - Who Covers The Employee Of The Ordering Party And Also Loved Ones (Partner/Spouse And Children). 2. Occupational Medicine For Employees Of The Employer: A) Insurance Coverage Includes The Organization And Covering By The Contractor The Costs Of Providing Medical Services In The Field Of Occupational Medicine, B) The Contract Is Carried Out By Providing Access By The Contractor With Access To Health Services And Their Implementation By The Service Provider, C) The Contractor Will Ensure The Possibility Of Performing Examinations In The Field Of Occupational Medicine At The Provincial Specialist Hospital.
