12.1. The Subject Of The Contract Is The Provision Of Gastronomic Services Consisting In Preparing Meals And Their Delivery By The Contractor To The Contracting Authority To Residents In A Social Welfare Home For Chronically Ill People In Lasowa Krowica In The Period From 01-07-2025 To 31-12-2025.12.1.1. The Agreement On This Order Will Be Concluded By The Nursing Home For Persons Of Mentally Patients In Lasowa Krowica, Lasowa Krowica 86, 37-625 Krowica Sam, Represented By The Director-Patrycja Mulawa12.2. At The Nursing Home For Persons Of Mentally Patients In Lasowa Krowica, Hereinafter Referred To As The Home Can Be Up To 28 Inhabitants (Currently 28 Inhabitants Are Staying). The Number Of Fed People Is Not Constant. The Number Of Inhabitants May Change Periodically, Due To Trips, Holidays, Stays In Hospitals, Etc. It May Also Be Reduced In A Natural Way. On Average, An Average Of 28 People Should Be Adopted To Calculate The Total Price Of The Nutrition For The Full Period Of Providing A Gastronomic Service. The Numbers Given Above Are Estimated Numbers. You Can Feed 28 Inhabitants A Maximum. The Actual Quantity Will Result From The Current Needs Of Orders Depending On The Number Of Residents Staying. The Contractor Will Be Entitled To Remuneration Only For Actually Supplied Meals. 12.3. The Gastronomic Service Is Performed Entirely In The Kitchen Rooms Of The Contractor And Using The Equipment In Them. For The Inhabitants Are Prepared Daily (7 Days A Week) Four Meals (For An Easy Diet With A Restriction Of Simple Carbohydrates - 5 Meals) At Established Seasons Of The Day. Meals For Residents Should Be Delivered Successively From: • Breakfast 7.00-7.45 • Lunch 12.30.-13.00 • Dinner 17.00-18.0012.4. Transport (Including Activities Related To Loading And Unloading) Of The Offered Foodstuffs Must Meet The Sanitary Requirements Regarding Food Transport Specified In The Provisions Of The Act Of 25 August 2006 On Food Safety And Nutrition (I.E. Journal Of Laws Of 2022 Item 2132). 12.5. The Employer Reserves The Right To Change Quantitative Deliveries Between Positions Regarding The Order With The Proviso That As A Result Of The Abovementioned Changes Will Not Be Exceeded, With The Contracting Authority Guarantees The Implementation Of The Subject Of The Contract At A Level Not Lower Than 60% Of The Size Of The Subject Of The Contract. If The Contracting Authority Exercises The Entitlement Referred To In The Preceding Sentence, The Contractor Will Be Entitled To Remuneration Due To Him Only Due To The Part Of The Subject Of The Contract To A Reduced Extent. The Implementation Of The Contract In The Remaining Scope Will Depend On The Actual Needs Of The Recipients Resulting From The Variable Number Of Those Entitled To Nutrition. The Price Specified In The Contract Is The Maximum Financial Limit To Which The Employer May Perform The Contract.
