The Subject Of The Contract Is The Provision Of A Persons Assistant Services In The Crisis Of Homelessness To Participants Of The Project Under Their Own Roof - Road To Independence, Implemented By The Social Assistance Center In The Period From July 1, 2025 To December 29, 2025 (Or From The Date Of Signing The Contract, If Its Signing Occurs After 01.07.2025 Until 29/12/2025) Services. 2. The Service Is To Be Provided By Two Assistants. 3. The Total Number Of Hours As Part Of The Order Will Be No More Than 375 Hours (Direct Support Under Services Is A Maximum Of 323 Hours, While Participation In The Project Team Meetings By 2 Assistants On Average 2 Times A Month After 2 Hours, Including 2 Hours During The Service, It Is A Maximum Of 52 Hours). The Minimum Guaranteed Number Of Hours Of Services That Can Be Performed By The Contractor Under This Agreement Is 263 Hours Of Services. 4. The Addressees Of The Benefits Are Adults. 5. The Contractors Are Not Entitled To Any Claims Against The Contracting Authority For The Non -Realization Of The Entire Subject Of The Contract Specified In Swz. 9. The Proposed Provisions Of The Contract, Constituting Annex 8, Contain, In Particular, Changes Provided For In Swz In The Form Of Unambiguous Contractual Provisions, Which Determine Their Scope And Nature And Conditions For Introducing Changes. 10. The Most Important Tasks Of The Assistant In The Crisis Of Homelessness Will Include, Among Others ,:1) Recognizing The Needs And Problems Of Project Participants; 2) Supporting The Implementation Of The Individual Assistance Plan And Social Contract, As Well As Periodic Monitoring And Evaluation Of The Plan With The Participation Of A Social Worker And Project Participant; 3) Support And Motivating The Participant To Take Specific Actions To Stabilize The Housing Situation; 4) Educating In The Field Independently Performing Activities Resulting From Living In The Premises, Including Cooking Simple Meals, Cleaning, Doing Washing, Dividing Responsibilities In Relation To Maintaining Order In Common Parts Between Participants Of The Project Inhabiting The Same Residential Premises; 5) Motivating To Obtain Permanent Income And Education In The Field Of Home Budget Management; 6) Assistance In Dealing With Official Matters, Medical Visits, Etc.; 7) Motivating To Care For Personal Hygiene; 8) Basic And Specialist Healthcare; 9) Motivating And Supporting In Building Relationships In The Local Environment, Except For The Environment Of People In The Crisis Of Homelessness; 10) Motivating And Supporting New Social Roles; 11) Education In The Field Of Interpersonal Skills;
