Tenders are invited for Procurement/ Installation of Information System Software and Networking for Finance/ Accounts Date Added: May 26, 2025 Deadline: 25/06/2025 Category: Medical Equipment, ICT and Software, Supply of Vehicles Interested companies are required to submit the following documents: i. Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Forms CAC 1:1 or CAC2 and CAC7; ii. Evidence of Companys Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all the Partners in case of Business Name) for the last three (3) years (2022, 2023 and 2024), valid till 31st December, 2025; iii. Evidence of current Pension (PENCOM) Compliance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 15 and above); iv. Evidence of Current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidders annual turnover is N50m and above); v. Evidence of current Nigerian Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2025; vi. Evidence of registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2025 or valid Certificate issued by BPP; vii. A Sworn Affidavit: a. Disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Yenagoa or the Bureau of Public Procurement is former or present Director, shareholder, or has pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars b. That none of the Companys Directors has been convicted in any country for any criminal offence including fraud or financial impropriety c. That the company is not in receivership, insolvency or bankruptcy viii. Reference letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed; ix. For Supply of Equipment: Letter of Authorization as representatives of the Original Equipment Manufacturers (OEMs); x. Works: List of Plants/ Equipment with proof of Ownership/ Lease; xi. Only Companies included in the National Automotive Design and Development Councils approved list of local manufacturers of vehicles or their representatives should bid for the Vehicles; xii. Companys Audited Accounts for the last three (3) years, 2022, 2023 & 2024 duly stamped by a reputable audit firm; xiii. Comprehensive company profile showing organizational structure with Curriculum Vitae of relevant key staff to be deployed for the project, including copies of their Academic and Professional Qualifications Tender Documents are obtainable from the Procurement Unit, Prof. E. N. Etebu Administration Block (2nd Floor), Federal Medical Centre, Yenagoa within the working hours of 8am-4pm (Monday Friday) upon presentation of evidence of payment of non-refundable Tender Fee of N10,000.00 only per LOT, paid into Federal Medical Centre, Yenagoa TSA at any reputable Commercial Bank in Nigeria Note: i. All Bids submission must be in English Language and signed by an official authorized by the bidder ii. All costs will be borne by the bidders iii. Bids received after the deadline for submission shall be returned unopened iv. Evaluation of Bids and documents shall be strictly based on the requirements specified above v. The Technical Bids will be opened in the presence of Bidders or their Representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional bodies. vi. Only pre-qualified bidders at technical evaluation shall be invited at a later date for financial bids opening, while the financial bids of unsuccessful bidders shall be returned unopened vii. All required documents may be referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue Service (FIRS), PENCOM, ITF and other relevant bodies for verification viii.
