This must be submitted in this manner: i. Detailed Company Profile with registered addresses, functional email, telephone numbers and point of contact for the organization. ii. Evidence of company statutory registration with Corporate Affairs Commission, including form CAC7 & CAC2 and any other business registration documents. iii. Tax Clearance Certificate for the last 3 years iv. Viable list of clients, including International Non-Governmental Organizations and other reputable organization equal in scope and Government agencies. v. Evidence of possession of experience in the line of field/category you are seeking prequalification. The evidence should include copies of completed Job Orders and Proof of Deliveries (Delivery Notes or Job Completion Notification/certificate) vi. Evidence of Financial Capability (Latest Audited Accounts). vii. Bank Reference Letter duly addressed to FHI360 Prequalification Committee viii. Evidence of Registration with Professional bodies, Regulatory Bodies, Manufacturers and/or Certificate of Authorized ix. Any other relevant supporting document that could aid your application.
