The Employer Requires The Delivery Of Equipment Produced After January 1, 2025. It Is Not Allowed To Offer Remodeling And Demonstration Medical Equipment. The Delivered Equipment Must Be Complete And Ready To Use Without Additional Purchases. A Detailed Description Of The Manner Of Implementation Of The Subject Of The Contract Was Specified In Annex 1A To Swz And The Draft Contract Constituting An Integral Part Of Swz. I. Description Of The Subject Of The Contract1. A Detailed Description Of The Subject Of The Contract Is Included In Annex 1 To Swz - Description Of The Subject Of The Contract. 1) The Contractor Is Obliged To Complete All Items In Annex 1 And 3 To Swz. 2) Delivery Includes: A) Purchase Of Medical Equipment - Yag/Slt Ophthalmology Laser - Hereinafter Referred To As Equipment; B) Assembly/Installation, Launch, Conducting Attempts At The Abovementioned Equipment; C) Training Of Designated Employees Of The Ordering Party In The Scope Of Service Of The Abovementioned Equipment; D) Training Of Technical Staff In The Field Of Technical Service And The Service Of The Abovementioned Equipment; E) Warranty Service In The Device, On The Principles Set Out In § 8 Contracts; F) Delivery Of The Warranty Card; G) Providing The Technical Passport Of The Abovementioned Equipment, H) Providing The Manual For The Medical Device In Polish, In Electronic Version In Polish And Technical Documentation (Service) .3. Medical Devices Constituting The Subject Of This Agreement Must Meet The Requirements Of The Current Legal Status, And In Particular Introduced By The Act Of 7 April 2022 On Medical Devices (Journal Of Laws 2024. 1620). 4. The Parameters Of The Standard Indicated By The Employer Present Technical, Operational, Utility, Functional And Other Features Relevant To The Subject Of The Contract, While The Indicated Brand Or Commercial Name Specifies The Product Class, Not A Specific Manufacturer. The Contractor May Offer Equivalent Products, Provided That They Have No Worse Technical Parameters Than Those Described And Given In Swz. 5. The Contractor Is Responsible For Quality, Compliance With The Technical And Qualitative Conditions Described For The Subject Of The Contract. 6. The Rules For The Implementation Of The Contract Are Set Out In The Draft Contract Constituting Annex 7 To Swz.
