The Subject Of The Contract Is The Provision Of The Municipal Municipal Services Board Of Municipal Services Of Corpse Corporation Along With The Transport Of The Corpse For Cremation And The Delivery Of The Urn With Ashes After The Demember Of The Deceased, Marked With A Plate With The Name And Surname To The Place Indicated By The Contracting Authority. The Contractor Is Obliged To Fulfill The Contract In Accordance With The Legal Provisions In Force In This Respect, In Particular In Accordance With The Act Of 31 January 1959 On Cemeteries The Dead (I.E., Journal Of Laws Of 2024, Item 576) And Executive Provisions Issued On Its Basis. Substantial Terms Of The Order: During The Implementation Of The Order, The Employer Will Respect 350 Cremation Services, Including About 10 Cremation Services Of A Corpse Of A Weight Above 140 Kg And About 3 Cremations Of Children Up To 5 Years Old). The Contractor Is Obliged To Perform The Order While Maintaining The Seriousness And Respect Due To The Deceased. The Cremation Service Will Be Performed On The Basis Of Unit Orders Issued By The Ordering Party. The Contractor Will Collect The Corpse For Cremation Within 24 Hours Of Placing The Order By The Ordering Party. The Contractor Will Deliver The Urn With Ashes Within 24 Hours Of Receiving The Corpse Into Cremation, Not Including Sundays And Holidays. The Reception Of The Corpse For Cremation Will Take Place From Monday To Friday (Excluding Holidays) From 7:00 To 18:00, After Prior Telephone Arrangement. The Place Of Receipt Of The Corpse For Cremation Is The Cold Room Of The Pre -Funeral House At Ul. Kozielska 120 In Gliwice. The Urns With Ashes Will Be Delivered From Monday To Saturday (Excluding Holidays) From 6:00 To 14:00, After Prior Telephone Arrangement. The Place Of Delivery Of The Urn Along With Ashes Is The Mzuk Funeral Plant At Ul. Kozielska 120 In Gliwice. The Contractor Will Receive The Corpse For Cremation In His Own Creative Coffin, Taking Into Account And Adapting The Casket To A Corpse Over 140 Kg / Below 140Kg And The Cremation Of Children Up To 5 Years Old. The Contracting Authority And The Corpse For Cremation Will Transfer The Contractor: Urn To Ashes, Photocopies Of The Death Certificate, Consent To The Cremation Of The Corpse. The Contractor Will Provide The Ordering Party: Urn With Ashes With A Plaque Stuck With The Name And Surname Of The Deceased, Cremation Certificate, Vat Invoice For The Service Performed. The Number Of Cremation Services During The Contract May Change In Accordance With The Actors Actual Demand. The Contracting Authority Guarantees The Contractor The Implementation Of An Order With A Value Of Not Less Than 50% Of The Total Value Of The Gross Contract Specified In §5 Para. 2. There Is No Claims Due To Reducing The Value Of The Contract.
