The Subject Of The Order Is The Delivery And Assembly Of The Cryotherapy Apparatus For The Task Entitled Purchase Of Medical Equipment For Hospital Lipno Sp.Z O.O. A Detailed Description Of The Subject Of The Contract Is Included In Annex 2 (Form Of The Required Parameters) To This Swz Constituting An Integral Part Of It. The Contractors Obligations Include:- Delivery And Transport,- Unloading And Bringing To The Rooms Indicated By The Ordering Party In The Hospital,- Launch And Training Of Staff (For The Equipment For Which It Is Required). The Contractor Undertakes To Further Train Staff, If It Is Necessary At A Later Date Agreed With The Employer. Conditions - The Requirements Specified By The Ordering Party In The Form Of The Parameters Required (Annex 2 To Swz) Must Be Met By The Contractor. Failure To Meet The Requirements Will Reject The Offer. In The Event Of The End Of Production Or Withdrawal From The Market, Which Is The Subject Of The Contract, A Change Is Allowed To A New Product With The Same Or Better Parameters. The Subject Of The Contract Offered By The Contractor Must Be Brand New In Accordance With The Specified Equipment (Annex 2 To Swz), It Must Be Complete And After Installation, Installation, Setting Ready For Work Without Any Additional Purchases And Investments. The Offered Subject Of The Contract Must Be Approved For Trading And Use In Health Care Units In The Territory Of The Republic Of Poland In Accordance With Polish Law And The Law Of The European Union. It Must Have Current Polish Or Applicable Certificates In The European Union, Certificates Of Admission To Use In Health Care Facilities If They Are Required In Accordance With The Law.
