Tenders Are Invited For Care And Health Services For Participants Of The Project Entitled Przystań Ploczayy In The Commune Of Chełmża Divided Into Tasks

Part 1: Care Services For Participants Of The Project Entitled Przystań Przystańyy In The Commune Of Chełmża, Provided By The Guardian/Guardian At The Place Of Residence Of The Participants Of The Project. 4.3.1. The Scope Of Tasks Of The Babysitter/Guardian: - Organizing Care And Social Support Towards The Person Indicated By The Center, - Exercising Care For The Charges/Charges In Order To Ensure Health Safety And Personal Hygiene, - Help In Everyday Activities, - Help In Everyday Activities - Motivation For Independence And Intellectual, Physical And Social Activity, - Preparation Of Meals, - Shopping. Keeping A Diary Of Care Services Towards Each Project Participant. 4.3.3. The Customer Will Receive The Service No Later Than 3 Days Before The Service Of The Service And Until The 25Th Day Of A Given Month, The Work Plan For The Service Of Providing The Service For The Next Month In Accordance With The Agreed Work Schedule Divided Into Individual Weeks. Any Change Related To The Provision Of Services Must Be Reported To The Project Coordinator And Accepted By Him. 4.3.4. Care Services Will Be Provided At The Place Of Residence Towards People Who Require Support In Everyday Functioning. The Employer Reserves That It Does Not Cover The Contractors Travel Costs To The Place Of Implementation Of The Subject Of The Contract. 4.3.5. Number Of Hours: Care Services Will Be Available Continuously 7 Days A Week In Relation To 6 Participants Of The Project Residing In The Commune At Their Place Of Residence. They Will Be Provided By 2 Babysitters/Guardians Of A Monthly Working Time For 1 Babysitter/60 H Guardian (On Average 15 Hours A Week/5 Hours Per 1 Person/Project Participant). 4.3.6. The Maximum Number Of Hours Of Services Rendered In The Period From 01.05.2025 To 30.04.2028 For 2 Babysitters Will Amount To A Total Of 4.320 Hours, While For 1 Babysitter/Guardian 2.160 Hours, An Average Of 120 Hours Per Month, 60 Hours Per Month (Falling For 1 Babysitter/Guardian). 4.3.7. The Above Specifies The Maximum Number Of Hours Of Support From A Given Area Throughout The Service Order. 4.3.8. The Contracting Authority Reserves That The Maximum Daily Number Of Services Rendered With One Project Participant Is Max. 3H. 4.3.9. The Number Of People And The Number Of Hours May Change And Cannot Be Determined Exactly Due To The Specificity Of The Order, Which Is Characterized By Variability Of Needs Due To The Changing Health And Life Situation Of People Who Require Help. Note: The Employer Reserves That The Services Will Be Provided On The Basis Of A Schedule For The Implementation Of Support Developed Taking Into Account The Individual Needs Of Project Participants. The Estimated Number Of Project Participants In The Given Months Will Be Variable And Determined By The People. Services Will Not Be Provided Simultaneously During The Contract For All Participants. The Support Schedule Will Be Determined According To The Demand Of Project Participants.
