Tenders Are Invited For Provision Of Services By Specialists As Part Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center-Edition 2.

1. The Subject Of The Contract Is The Implementation Of Specialized Therapeutic Classes With Children With Disabilities Or Endangered Disability, As Part Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center In Będzin, Divided Into The Following Parts Of The Following Parts Mentioned In The Field Of Individual Therapies: Part 1.-Pedagogical Therapy-200 Hours, Including A Maximum Of 8 Hours. Consultations; Above The Maximum Number Of Hours In Each Part Is Given. The Occurrences Are Specified In Art. 90V Para. 4 Of The Act Of 7 September 1991 On The Education System And In The Regulation Of The Minister Of Education And Science Of August 30, 2023 On Detailed Tasks Of Leading Coordination, Rehabilitation And Care Centers (Journal Of Laws Item 1801). The Main Goal Of The Leading Coordination, Rehabilitation And Care Centers Is To Provide Comprehensive Support To Families With Children With Disabilities And Endangered Disability. 3. The Tasks Of The Leading Coordination, Rehabilitation And Care Center Are Financed From The State Budget. 4. Therapeutic Classes Will Be Implemented In: 1) In The Rooms Of The Special Educational Center Lub2) Depending On The Childs Needs, In His Family Home/Place Of Permanent Residence. 5. The Contractor Who Will Be Elected For A Given Part Of The Order Will Be Obliged To Remain Ready To Perform The Service Throughout The Contract. The Implementation Of Classes Will Depend On The Number Of Applicants And Qualified For Specialist Help Of Children. 6. The Contractor Will Provide Services In Hours And Place Agreed With The Employer (After Agreeing With The Parents/Guardians Of Children) - From Monday To Friday, From 7.00 To 19.00, With The Proviso That During The Holiday Season The Implementation May Take Place From 8.00 To 14.00.7. The Schedule Of Classes And The Allocation Of Children For Therapeutic Classes Will Be Determined After The Childrens Appointment And Qualifying Them To Specific Forms Of Therapy. 8. The Contractors Duties Will Include: 1) Conducting Early Support For The Childs Development According To His Psychophysical And Developmental Needs; 2) Conducting Therapeutic Classes With Disabled Children And Endangered Disability In Accordance With Their Qualifications; 3) Providing Comprehensive Assistance To Children And Families From The Moment The Threat To Disability Is Detected, With Particular Emphasis On Children Up To 3 Years Of Age; 4) Developing A Functional Diagnosis Of A Child And A Comprehensive Therapy Program, Including Individual Programs For The Development Of A Child Or Individual Therapy Programs; 5) Making A Systematic Assessment Of The Effectiveness Of Impacts; 6) Cooperation With Other Specialists Working With The Child In Order To Increase The Effectiveness Of Therapy. 9. A Detailed Description Of The Subject Of The Contract And Detailed Conditions For Its Implementation Were Included In The Proposed Contractual Provisions (Annex 5 To Swz).
