A Plaque For Testing Visual Acuity - 1 Pc. 8) Padnom For Learning To Self -Examine Breast - 1 Pc) Pressure Holter - 2 Pcs. 10) Holter Ecg With Software - 1 Pcs. 11) Spirometr - 1 Pc To Swz - A Form Submitted By The Contractor Together With The Offer And Designed Provisions Of The Contract, Which Will Be Introduced Into The Content Of The Concluded Contract Constituting Annex 6 To Swz. The Parameters Described In Annex 2 To Swz As The Required Technical And Utility Parameters Are A Detailed Description Of The Subject Of The Contract, The Failure To Reject The Offer As Incompatible With The Terms Of The Contract Pursuant To Art. 226 Para. 1 Point 5 Of The Public Procurement Law. The Offered Medical Equipment Must Have The Required Certificates, Certificates And Marking Provided For By The Provisions Of The Act Of 7 April 2022 On Medical Devices (Consolidated, Journal Of Laws Of 2024, Item 1620, As Amended), Proving The Required Approval For Use And Trading In Poland, I.E. Ce Certificates And Declarations - Not For A Product That Is Not A Medical Service In The Meaning Of The Act On The Act Medical Devices. The Contractor Is Obliged To Provide A Model/ Type To Identify The Offered Subject Of The Contract. The Model/ Type Of Equipment Offered Is An Important Element Of The Content Of The Offer For The Employer Due To The Need To Determine The Compliance Of The Content Of The Offer With Swz, I.E. Comparisons Of Equipment Offered By The Contractor With The Parameters Required By The Contracting Authority. Lack Of Entry Will Result In Rejection Of The Offer As Incompatible With The Specification.
