The Subject Of The Contract Is The Implementation Of Tasks In The Field Of Government Administration, Resulting From The Comprehensive Support Program For Families For Life, Which Is An Attachment To Resolution No. 189 Of The Council Of Ministers Of 27 December 2021 On The Program Of Comprehensive Support For Families For Life (M.P. 2022 Item 64), Hereinafter Referred To As The Program In The Implementation Of The Tasks Of The Leading Center Coordination, Rehabilitation And Care In The Lublin Poviat, Referred To In Art. 90V Para. 4 Of The Act Of 7 September 1991 On The Education System (Journal Of Laws Of 2024 Item 750) And The Regulation Of The Minister Of Education And Science Of 30 August 2023 On Detailed Tasks Of Leading Coordination, Rehabilitation And Care Centers (Journal Of Laws Of 2023 Item 1801) Resulting From Task 2.4, Priority Ii Of The Program: Comprehensive Assistance To A Disabled Child From O. To Start School And His Family For People Qualified In The Scope Of Part 1 - Classes In The Field Of Speech Therapy/ Neurologopedia (Maximum 170 Hours). The Detailed Description Of The Subject Of The Contract Is Set Out In Annex 1 To Swz. The Contractor May Submit An Offer For All Parts Of The Order, Provided That The Person Conducting The Classes Will Only Be Once Indicated In A Given Part Of The Order. This Means That One Person May Conduct Classes In Speech Therapy/Neurologopedia In Only One Part Of The Order (Provided That The Required Qualifications And Experience). In The Event That One Person Is Indicated For More Than One Part Of The Order, All Contractor Offers Submitted To These Parts Of The Order Will Be Rejected.
