Tenders Are Invited For Successive Delivery Of Tests, Packaging And Consumables For Sterilization, Respectively From One To Two Parts Of The Order

1. The Subject Of The Contract Is The Successive Delivery Of Accessories For The Sterilization Process. 2. The Offered Goods (Item 3.1 Swz) Must: 2.1. Meet The Conditions For Marketing And For Use, Provided For In The Act On Medical Devices Of April 7, 2022 (Journal Of Laws 2022, Item 974, As Amended - Hereinafter: Act On Medical Devices), Confirmed By Current Documents (I.E. Complete Notifications Or Notifications To The President Of The Office For Registration Ce, Declarations Of Conformity, Approvals) - For Those Goods That, In Accordance With Applicable Law, Require This; 2.2. Have Documents Allowing Them To Trade And Use In Poland - Including Ce Certificate And Documents Confirming That These Goods Meet The Standards Required By Law - In The Case Of Goods Not Subject To The Provisions Of The Act On Medical Devices Or Pharmaceutical Law; 2.3. For Products Qualified As Cosmetics In Accordance With The Regulation Of The European Parliament And Of The Council / Ec / No. 1223/2009 Of November 30, 2009, Have A Notification On The Cosmetic Notification Portal (Cpnp Portal); 2.4. Be New, Devoid Of Flaws, Packaged In The Original Packaging. The Labels And Packaging Of The Goods Should Include All The Required Information In Accordance With The Act On Medical Devices, Pharmaceutical Law Or Other Regulations Of Generally Applicable Law; 2.5. Have A Validity Period Of A Minimum Of 12 Months From The Date Of Its Delivery - In The Case Of Goods With A Limited Expiry Date; 2.6. For Part 1 Of The Order, Item: From 1 To 6 In The Table Posted In Sopz (Annex 3 To Swz), The Ordering Party Requires That The Subject Of The Contract Has A Declaration Of Ce Compliance Issued By The Manufacturers Sleeves Confirming Compliance With The Norm: Pn-En Iso 11607-2, Pn-En 868-5. This Requirement Should Be Confirmed By Relevant Documentation, In Accordance With The Provisions Of Point 8.1.2 Swz.3. The Contractor Is Obliged To Absolutely Guarantee The Fulfillment Of The Quality Conditions Set Out In The Production Permit Or Other Decisions Determined By The Ministry Of Health, Based On Which The Goods Were Allowed To Market In The Republic Of Poland And Compliance With Expiration Dates For The Delivered Goods. 4. In The Case Of Each Part Of The Order Referred To In Point 3.1 Swz, The Contractor Is Obliged Without Additional Fees To: 4.1. Supplying Goods Successively To The Headquarters Of The Ordering Party (Ul. Montelupich 4, Kraków), Based On Orders Determining The Range And Quantity, In No Longer Than Within 5 Business Days From The Moment Of Placing The Order, With The Proviso That The Delivery Will Be Taken On Business Days (From Monday To Friday, Excluding Public Holidays), From 8:00 To 14:00. The Concept Of Working Days Is Understood As Monday To Friday, Excluding Public Holidays; 4.2. Bringing And Discharging The Goods In The Room (Lok. 03) At The Headquarters Of The Ordering Party. 5. A Detailed Description Of The Subject Of The Contract (Sopz) Is Annex 3 To Swz And
