The Implementation Of The Order Was Specified In The Proposed Contractual Provisions - Annex No. 5 To Swz - Draft Contract. The Term For Delivery Understands Cyclical Deliveries Carried Out During The Period Of The Contract, In Accordance With Orders Placed By The Contracting Authority, Adapted To His Current Needs. All Offered Medical Materials That Are Medical Devices Must Be Placed On The Market And Use On The Territory Of The Republic Of Poland, Classified In Accordance With Applicable Regulations. The Offered Products Must Be Marked With The Ce Mark, Have Instructions For Using In Polish And Be Marked With A Trade Name/ Name Or Reserved Trademark, The Address Of The Registered Place Of Conducting The Producers Activity. If The Registered Place Of Conducting Production Activities Is Outside The Eu Have The Name Of The Authorized Representative And The Address Of His Registered Place Of Business.
