Tenders Are Invited For Successive Provision Of Health Services In The Field Of Laboratory Diagnostics At The Request Of Sp Zoz In Bogatynia Together With The Supply Of Specific Substrates

1. The Subject Of The Contract Is The Successive Provision Of Health Services In The Field Of Laboratory Diagnostics At The Request Of Sp Zoz In Bogatynia In Accordance With The List Of Tests Required By The Contracting Authority. The List Of Laboratory Tests Is Attached As Annex Ii.2 To This Swz. 2. A Detailed Description Of The Subject Of The Contract - A Description Of The Border Conditions For Laboratory Tests Is Annex Ii.1 To This Swz. 3. The Contractor, Whose Offer Will Be Selected As The Most Advantageous By Way Of This Procedure, Will Present To The Ordering Party A List Of Tests That Can Be Performed Above The Border Scope And Will Become This List To An Appropriate Attachment To The Contract Selected By Way Of These Proceedings. 4. The Contracting Authority Requires That The Contractor Ensure For 24 Hours The Performance Of Laboratory Tests Listed In The List Of Required Tests - Border Range Constituting Annex Ii.2 To Swz And The Subsequent Contract. 5. The Contractor Must Perform The Subject Of The Contract With The Participation Of An Employed Qualified Medical Staff - Laboratory Diagnostics, Entered On The List Of The National Chamber Of Laboratory Diagnostics (Kidl) And Medium Staff Meeting Professional Requirements In The Scope Of This Order. 6. The Contractor, In The Scope Of Performance Of The Order, Must Have The Necessary Laboratory Equipment, Which Has All The Current Required Approvals And Certificates Of Admission To Work In The Medical Diagnostic Laboratory. 7. The Material For The Examination Will Be Taken By The Employers Staff At Its Expense At The Agreed Hours. This Material Will Be Properly Prepared For The Contractor According To The Recommendations, The Requirements Described By The Contractor And Presented In The Offer In Accordance With The Relevant Provisions Of The Contract, I.E. Annex Vii To Swz And The Subsequent Contract Selected Through This Proceeding. 8. The Contracting Authority Requires That The Contractor Receive The Material For The Examination From Monday To Friday, With The Exception Of Holidays By The Authorized Contractor From 11:00 To 12:00 From The Sp Municipal Hospital Of Sp Zoz In Bogatynia At Ul. Szpitalna 16. 9. The Agreed Order For Paper Tests Will Apply. In The Case Of Connection To The It System, The Employer Will Agree On The Form Of Proceedings With An Electronic Order. The Contractor Will Provide Specific Orders Of The Contracting Authority Throughout The Entire Period Of The Contract:- Substrate Breeding Substrates;- Transport-Proofs For Breeding Oxygen Bacteria And Relative Toaeroba Bacteria; According To The Schedule Of Substrate Supply, Which Will Be An Attachment To The Contract Selected Through This Proceeding. This Schedule Will Be Supplemented Before Signing The Contract Selected By Way Of This Proceeding. 10. The Receipt Of Collected Materials For The Test And Providing Test Results Lies In The Duties Of The Contractor, Will Take Place Thanks To His Efforts And At His Expense. 1
