Education And Training Services - Conducting Training In The Field Of Honorary Blood Donation Promotion For Public Blood Service As Part Of The Health Policy Program Entitled Ensuring The Self-Sufficiency Of The Republic Of Poland In The Blood And Its Components For 2021-2026. I. Determining The Subject Of The Contract: The Subject Of The Contract Is The Service Consisting In Organizing And Conducting The Training Entitled: The Use Of Storytelling And Person To Effectively Reach And Involve Participants In Activities In The Field Of Promotion Of Honorary Blood Donation. The Training Aims To Equip Participants With Knowledge And Skills In Creating Engaging Narratives And The Use Of Persons, Which Will Allow You To Effectively Reach Potential Blood Donors And Motivate Them To Undertake An Activity Promoting Honorary Blood Donation And In Accordance With All The Requirements Set Out In The Descriptions Of The Subject Of The Contract, Which Constitute Attachments To Swz As Part Of The Implementation Of The Health Policy Program Of The Minister Of Health Minister. Ensuring The Self-Sufficiency Of The Republic Of Poland In The Blood And Its Components For 2021-2026. Pursuant To Art. 257 Of The Pzp Act. The Contracting Authority Provides For The Possibility Of Annulment Of The Public Procurement Procedure If The Funds That The Employer Intended To Allocate To Finance All Or Part Of The Contract Was Not Granted To Him. The Order Must Be Processed In Accordance With The Requirements Set Out In This Swz And The Planned Provisions Of The Contract (Hereinafter Ppu). Ii. In Question: Pursuant To Art. 105 And 106 Of The Public Procurement Law In Order To Confirm The Compliance Of The Offered Deliveries With The Requirements, Features Or Criteria Set Out In The Description Of The Subject Of The Contract Or Criteria For Assessing Offers Or Requirements Related To The Implementation Of The Order, The Contracting Authority Requests The Contractors To Provide The Following Documents: 1. A Detailed Training Schedule Together With An Indication Of The Date And Place Of Implementation. 2. Declaration Of The Contractor With A List Of Main Trainers (4 People) And Reserve Trainers (Minimum 2 People) - Containing All The Information Required And Described In Point 2.2 Of This Swz And In The Description Of The Subject Of The Contract (Opz) Together With The Assurance That The Trainers Indicated In The Statement Will Be Acquainted With The Specifics Of Work At The Blood Donation Center Related To The Propagation Of Honorary Blood Donation. 3. Pursuant To Art. 107 Para. 1 Of The Public Procurement Law, The Contractor Is Obliged To Submit The Above -Mentioned Evidence Together With The Offer And It Should Exercise Every Diligence So That These Documents Are Complete And Contain All The Necessary Information Enabling The Contracting Authority To Verify Them In Terms Of Regularity. 4. Ordering Party In Accordance With Art. 107 Paragraph 2 Of The Public Procurement Law Provides For The Possibility Of Completing The Abovementioned In Question Within The Time Limit Set By The Ordering Party. 5. The Subjective Measures Referred To Above Are Submitted In The Manner Provided For In The Provisions Issued Pursuant To Art. 70 Of The Pzp Act. 6. Documents Prepared In A Foreign Language Are Submitted Along With The Translation Into Polish. 7. Pursuant To Art. 107 Para. 1 Of The Public Procurement Law, The Contractor Is Obliged To Submit The Above -Mentioned Evidence Together With The Offer And It Should
