The Subject Of The Contract Is The Provision Of Medical Services In The Field Of Occupational Medicine Consisting In: 1) Ensuring Preventive Healthcare For Work Candidates And Employees Of Eko Dolina Sp. Z O.O., Based On The Provisions Of The Act Of June 27, 1997. On The Service Of Occupational Medicine (Vol. Journal Of Laws 20122.437) And The Regulation Of The Minister Of Health And Social Welfare Of May 30, 1996. On Conducting Medical Examinations Of Employees, The Scope Of Preventive Healthcare For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code (I.E. Journal Of Laws 2013.607); 2) Cooperation With The Employer In Fulfilling His Duties In The Field Of Occupational Health And Safety, Through Participation In Reviews Of Working Conditions, During Which Advice Will Be Given In The Organization Of Work, Ergonomics, Physiology And Work Psychology, Based On The Provisions Of The Act Of 27 June 1997. On The Service Of Occupational Medicine (Vol. Journal Of Laws 20122.437). 2. The Scope Of The Subject Of The Contract Includes Services Consisting In: 1) Performing Preliminary, Periodic And Control Of Employees, Together With The Provision Of Medical Services Including Laboratory And Diagnostic Tests To The Extent Necessary To Issue Medical Certificates And Specialist Consultations; 2) Performing Medical Examinations Aimed At Deciding On The Possibility Of Performing Previous Work In The Event Of Reporting By The Employee The Need For Such Examination Periodic, Including Preventive Ophthalmological Examinations Of The Employee In The Event Of A Deterioration Of The Employees Eyesight, Completed With The Issuing Of A Decision By An Occupational Medicine Doctor; 3) Performing Medical Examinations In The Event Of The Employees Certificate Of Disability Certificate; 4) Performing Medical Examinations In The Event Of A Change In The Employees Position; 5) Performing Psychotechnical Examinations, Among Others. For Drivers, Operators, Security Staff, Persons Performing Work At Height; 6) Providing Protective Vaccines And Vaccinations, Tracking The Terms Vaccination And Informing The Contracting Representative About The Need To Perform Them For At Least 7 Days Before The Date Of Their Performance; 7) Issuing Medical Certificates For The Purposes Set Out In The Labor Code And Executive Provisions Stating There Is A Lack Of Health Contraindications To Work In A Specific Workplace Or Counterparting. Healthy To Work In A Specific Position, Including The Need To Work With Eyeglasses To Work To Work While Servicing Electronic Screen Monitors Issued By An Ophthalmologist And Prescriptions For Glasses; 8) Participation Work On The Preparation (Update) Of Occupational Risk Assessments, Consultation In The Field Of Topics Related To Accidents At Work And Working Conditions; 10) Participation Of An Occupational Medicine Doctor In The Work Of The Bhp.3 Committee.
