1. The Subject Of The Basic Contract Is The Provision Of Services In The Field Of Occupational Medicine Together With A Medical Package For Employees Of The Chancellery Of The Polish Academy Of Sciences, Then Chancellery Of The Polish Academy Of Sciences And Mr. Irwir In Places Where The Polish Academy Of Sciences Has The Headquarters Of The Polish Academy Of Sciences, I.E. Gdańsk, Katowice, Kraków, Lublin, Łódź, Olsztyn, Poznań, Warsaw, Wrocław - In 12 Months. The Description Of The Subject Of The Contract Was Specified In A Detailed Description Of The Subject Of The Contract (Sopz), Constituting Annex 1 To Swz And In The Planned Provisions Of The Contract Constituting Annex 7 To Swz.3. The Employer Does Not Reserve The Possibility Of Applying For The Award Of The Contract Only By The Contractors Referred To In Art. 94 Of The Pzp.4 Act. The Contracting Authority Requires That The Contractor And His Subcontractors Employ A Person Performing Activities Involving Contacting The Employer And Providing Any Explanations Related To The Performance Of The Contract For The Provision Of Medical Services Acting As A Clients Guardian On The Basis Of An Employment Relationship, Within The Meaning Of The Provisions Of The Act Of 26 June 1974 - Labor Code (I.E., Journal Of Laws Of 2025, Item 277). 5. Requirements For The Implementation And Enforcement Of The Employment Requirement On The Basis Of An Employment Relationship Were Specified In The Planned Provisions Of The Contract, Constituting Annex 7 To Swz.6. The Employer Reserves The Right To Exercise The Right To Extend The Contract For The Execution Of The Contract For No More Than 12 Months From The Performance Of The Basic Contract, Subject To The Change Of The Deadline Referred To In § 9 Para. 12 Of The Planned Provisions Of The Contract, Constituting Annex 7 To Swz. The Option Of The Option Consists In Providing Medical Services Of The Same Type And Scope As The Basic Order Described In Sopz, Constituting Annex 1 To Swz, With The Remuneration Of The Monthly Flat Rate Indicated By The Contractor In The Offer, Which Will Constitute Annex 2 To The Contract. The Option Of The Option Is The Unilateral Right Of The Contracting Authority, From Which He May, But There Is No Obligation To Use As Part Of The Implementation Of The Subject Of The Contract. If The Contracting Authority Fails To Take Advantage Of The Contractors Right, No Claim Is Entitled To. If The Contracting Authority Uses The Order With The Option, The Contractual Obligation Is The Contractor To Perform The Benefit Within The Scope Covered By The Order With The Option. 7. The Employer Does Not Specify The Additional Requirements Related To Employing The Persons Referred To In Art. 96 Para. 2 Point 2 Of The Public Procurement Law. 8.
