The Subject Of The Contract Is The Modernization Of Spzzoz In Żuromin Covered By Co -Financing From The Government Polish Fund Order: Strategic Investment Program - Edition 8 In The Following Scope: 1) Action 1 - Purchase Of The Equipment Of The Operating Theater And The Anaesthesiology And Intensive Care Department In Spzzoz In Żuromin, Including: Devices For Anesthesia, Specialist Beds With Matters And Hangers For Giving Matters And Hangers For Servants And Hangers Drips And Laparoscopic Tools, Which Will Be An Integral All Of Your Equipment In The Operating Theater And Will Complement It. The Scope Of This Action Includes: A) Purchase Of Equipment/Equipment For The Rooms Of The Operating Theater And The Anesthesiology And Intensive Care Department At Spzzoz In Żuromin In The Assortment, Quantities And In Accordance With The Requirements Of The Ordering Party Specified In The Description Of The Subject Of The Contract Constituting Annex No. 12.1B) The Offered Subject Of The Contract Must Be Factory -New, Free From Defects, Use In The Republic Of Poland And Use In Health Care Facilities In Accordance With The Act Of April 7, 2022. On Medical Devices (Journal Of Laws Of 2024, Item 1620) - A Declaration Of Compliance Marked With A Ce Mark For Medical Devices And Be Compatible With The Standards In Force In The European Union C) The Contractor Is Obliged To Provide Equipment For Spzzoz In Żuromin, Mount Equipment And Train Staff In Use - If It Concerns, Both Training Contractors, Transport Is Organized By The Contractor. The Risk Of Accidental Loss Or Damage To The Equipment/Equipment Passes At The Contracting Authority As Soon As He Is Delivered To The Place Of Collection And Takeover By The Contracting Authority. 2) Measure 2 - Modernization Of The Premises Of The Clinic Building Along With The Purchase Of An External Elevator In Accordance With The Construction Design And A Building Permit Decision. The Scope Of This Action Includes: A) Execution Of All Works Necessary To Modernize The Rooms And The Purchase And Installation Of An External Elevator, Including Construction Works; B) Preparing Complete As -Built Documentation; C) Performance At The Expense Of The Contractor All Necessary Activities And Obtaining All Necessary Documents Necessary To Obtain A Decision For Use; D) Obtaining The Final Decision On The Permission To Use The Contracting Authority. 3) Measure 3 - Renovation And Retrofitting Of: Gynecology And Obstetrics Together With The Delivery, Neonatal, Rehabilitation, And Childrens. The Scope Of This Action Includes: A) Development Of The Concept And Project Documentation On The Basis Of A Functional And Utility Program Together With The Application For Works/Obtaining A Building Permit In The Event That It Concerns And All Other Administrative Decisions, Arrangements, Consents And Opinions Necessary To Implement The Subject Of The Contract Andb).
