Tenders Are Invited For Provision Of Sanitary Transport Services To The Municipal Hospital In Ruda Śląska Sp. Z O.O.

1. The Subject Of The Contract Is The Provision Of Sanitary Transport Services For The Municipal Hospital In Ruda Śląska Sp. Z O.O. A Detailed Description Of The Subject Of The Contract Is Included In Annex 1 To Swz/Contract, In Annex 4 To Swz/Annex 6 To The Contract And In Annex To 9 To Swz.2. Pursuant To Art. 95 Para. 1 Of The Act, The Employer Requires That Persons Performing Activities Directly Related To The Service (I.E. Drivers Entitled To Drive Vehicles That Will Use During The Performance Of The Service Under The Performance Of This Contract) Are Employed Under An Employment Contract. 3. Requirements Regarding The Method Of Documenting Employment Of A Person Under An Employment Contract, Control Rights Of The Contracting Authority And Sanctions For Failure To Meet The Requirements Referred To In Point 2 Above Were Specified In Detail In The Planned Provisions Of The Contract, Which Will Be Introduced Into The Content Of The Contract On The Contract (Annex No. 9 To Swz). 4. Whenever The Order Documents Refer To Polish Standards, It Should Be Understood As Polish Standards Transferring European Standards Or The Standards Of Other Member States Of The European Economic Area Or Other Standards Or Documents Referred To In Art. 101 Para. 1 Point 2 Of The Act. Where In The Description Of The Subject Of The Contract There Was An Indication Of Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Para. 1 Point 2 And 3 Of The Public Procurement Law, The Contracting Authority In Accordance With Art. 101 Para. 4 Of The Pzp Act Allows The Submission Of Bids Containing Equivalent To Described, In Relation To Their Quality, Target Purpose And Fulfilled Functions And Utility Values. The Contractor May Refer In The Offer To The Use Of Equivalent Solutions Described In Swz, Submitting A List Of Equivalent Solutions. In This Case, The Contractor Is Obliged To Show That The Solutions He Offers Meet The Requirements (Equivalence Criteria) Specified By The Contracting Authority And Will Be Marked As Equivalent. In Terms Of Solutions That Are Proposed As Equivalent - The Contractor Should Prove In The Offer, In Particular By Means Of The Evidence In Question Referred To In Art. 104 - 107 Of The Act That The Proposed Solutions Correspond To The Required By The Ordering Party. The Contracting Authority Requires At The Stage Of Conducting A Request By The Contractor For Admission Of An Equivalent Solution. The Contractor Who Applies Equivalent Solutions Will Be Obliged To Demonstrate During The Performance Of The Contract At Each Request Of The Ordering Party That The Solutions He Applied To Balance Meets The Requirements Set By The Contracting Authority In The Description Of The Subject Of The Contract.
