If applying for different supplies and services from the above categories, A, B, C, D, E and F separate applications should be submitted for the same. The top left corner of the envelope containing the application should be clearly marked as Registration of Suppliers for Sampath Centre 2025 - 2027. Cheques of Rs.6000/- (none-refundable) drawn in fever of Sampath Centre (for each section) or the receipt obtained by paying the relevant money to the Sampath Centre should be attached with the application and sent it to the address, Manager, Sampath Centre, No.110, Sir James Peiris Mawatha, Colombo 02 on or before 30th day of April 2025.
