Health Services - Digital Aftercare (Mental Illnesses). Insured Persons With Serious Mental Illnesses Are To Be Provided With A Nationwide, Non -Residential Digital Aftercare Offer. The Legal Basis For The Contractual Agreement With A Future Contractor Is The Paragraph §140A Sgb V. The Offer Is Aimed At Adults From The Age Of 18, Which Are Released From The Hospital For The First Time With Defined Psychological Indications (Depression, Schizophrenia). The Offer Is Based On A Digital/Hybrid Supply Concept Consisting Of Video Telephone Conversations (Tele Coaching) And An App Or Web-Based Application For Insured Persons. The Actual Individual Needs Are Determined Within The Offer As Part Of An Assessment. No (Digital) Psychotherapy Is Provided As Part Of The Offer. Goals: The Aftercare Offer Helps To Close An Existing Supply Gap. Previous Therapeutic Successes Should Be Secured In The Long Term, Insured Persons Are Supported And Accompanied In The Transition To Outpatient Regular Care. Learning Strategies To Deal With Impending Crises Should Be Avoided Again. The Scope Of Services Is Approximately 2,000 Care Per Year, The Maximum Amount Is 2,500 Care Per Year. The Planned Go-Live Is In Q3/2025.
