Work Detail

1. The Subject Of The Contract Is: Delivery Of An Ultrasound Apparatus, I.E. High -Class Ultrasound Designer For Obstetric And Gynecological Tests Together With The Assembly And Starting Of The Camera (Hereinafter Referred To As The Camera). Delivery Of The Camera On The Basis Of An Operational Lease Agreement With The Option Of Redemption. 2. Justification Of The Lack Of Division Into Parts. In View Of The Estimated Value Of The Order, It Can Be Realized Even By A Micro/Small/Medium Entrepreneur. Bearing In Mind That The Law Of Entrepreneurs As A Micro Entrepreneur Recognizes An Entity That, In At Least One Of The Last Two Financial Years, Employs On Average Annually Less Than 10 People And Has Achieved A Annual Net Turnover From The Sale Of Goods, Products And Services As Well As Financial Operations Not Exceeding Equivalent In Pln 2,000,000, Or The Sum Of Assets Of Its Balance Sheet Prepared At The End Of One Of These Years Did Not Exceed The Equivalent Of Pln 2,000 Eur 000, The Contracting Authority Decided That The Scale Of The Order Enables Applying For An Order To Be Awarded Even Micro Entrepreneurs Also If The Contract Is Not Predicted In Parts. Therefore, The Division Of The Order Into Parts Would Not Expand The Circle Of Entities That Could Take Part In It. According To The Reasons For Directive 2014/24/Eu Of The European Parliament And Of The Council Of 26 February 2014 On Public Procurement, Repealing Directive 2004/18/In One Of The Objectives Of The Current Directive, It Was Easier To Facilitate The Participation Of Small And Medium -Sized Enterprises In Public Procurement. 3. A Detailed Description Of The Order Can Be Found In Annex 1A To Swz.4. The Contracting Authority Requires That The Ultrasound Apparatus To Be Factory (Year Of Production Not Earlier Than 2024), Free From Defects. The Ordering Party Does Not Allow The Use Of A Used Or Recondient Camera, Or Previously Used By Another User, Including In Particular The Apparatus Used To Provide Health Services - This Also Applies To All Components, Equipment, Accessories, Etc. Along With The Delivery, The Contractor Will Submit A Photocopy Of The Document Allowing The Offered Camera, In Accordance With The Act Of 7 April 2022 Medical *Consolidated: Journal Of Laws From 2024 Item 1620). 5. The Offered Subject Of The Contract Must Meet The Requirements Set Out In (If Concerned): A) The Act Of 7 April 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620), B) Regulation Of The Minister Of Health Of 05.11.2010 On The Method Of Classifying Medical Devices (Journal Of Laws No. 215, Item 1416), C) Of The Minister Of Health Of February 17, 2016. On The Basic Requirements And Procedures For Assessing The Compliance Of Medical Devices (Journal Of Laws Item 211), D) Regulation Of The European Parliament (Eu) 2017/745 Of 5 April 2017 On Medical Devices. 4. The Contractor Is Obliged To Complete The Order On The Principles And Conditions Described In The Proposed Provisions To The Contract
