1. The Subject Of The Contract Is The Provision Of Occupational Medicine Services For Employees Of The Nursing Team Of The Capital City Of Warsaw Warsaw. 2. The Subject Of The Contract Includes: 1) Conducting Preventive Examinations Of The Employers Employees: Preliminary, Periodic And Control As Well As Sanitary And Epidemiological Research Provided For In: A) The Act Of 26 June 1974 Labor Code (I.E., Journal Of Laws Of 2023, Item 1465, As Amended-Hereinafter: Kp) Infectious Diseases In Humans (I.E., Journal Of Laws Of 2024, Item 924 - Hereinafter: The Act), C) The Regulation Of The Minister Of Health And Social Welfare Of 30 May 1996 On Conducting Medical Examinations Of Employees, The Scope Of Preventive Health Care Over Employees And Medical Decisions Issued For The Purposes Provided For In The Labor Code, In Accordance With The Order Issued By The Ordering Order (I.E. Dz. U. From 2023, Item: Regulation), 2) Performing Laboratory Tests Commissioned By An Occupational Medicine Doctor, 3) Issuing Medical Certificates For The Purposes Set Out In The Labor Code And Executive Provisions, In Accordance With Applicable Patterns, In Two Copies, In Two Copies, 4) Participation Cited In The Manner Specified In Art. 23712 Kp, 5) Keeping And Storing Medical Records In The Field Of Preventive Healthcare In Accordance With Applicable Regulations, 6) Preparation And Storage Of Medical Records In Accordance With The Regulation Of The Minister Of Health Of July 29, 2010 On The Types Of Medical Documentation Of The Occupational Medicine Service, How To Conduct And Storage, And Patterns Of The Documents Used (Journal Of Laws No. 149, Item 1002). The Contracting Authority Defines Employment Requirements On The Basis Of An Employment Relationship, In The Circumstances Referred To In Art. 95 Of The Pzp Act: 1) The Contracting Authority Requires Employment Under An Employment Contract Of At Least ½ Full -Time By The Contractor Or Subcontractor Of Persons Performing The Activities Indicated Below During The Performance Of: Administrative Activities (Patient Registration) .2) The Contracting Authority Requires That Employment To Perform The Contract Lasts Throughout The Contract, In The Event Of A Termination Of The Employment Relationship By The Person Employed Or By The Contractor / Subcontractor Before The End Period, The Contractor / Subcontractor Will Be Obliged To Employ Another Person In This Place. 3) Detailed Conditions For The Implementation Of The Requirements Set By The Ordering Party Pursuant To Art. 95 And 438 Of The Public Procurement Law, In Particular The Method Of Documenting Employment, The Contracting Authoritys Rights In The Scope Of Controlling The Contractors Fulfillment Of The Requirements Referred To Above And Sanctions For Failure To Meet These Requirements, Are Included In The Planned Contractual Provisions Constituting Annex 10 To Swz.
