The Subject Of The Contract Is The Provision Of A Service In The Field Of Medical Waste Management Of The Municipal Hospital In Ruda Śląska Sp. Z O.O. A Detailed Description Of The Subject Of The Contract In The Scope Of The Service Of The Service Was Specified In Annex 1 To Swz/Contract And Annex 9 To Swz. 2. Services Should Be Performed In Accordance With Applicable Regulations, In Particular: 1) Of The Act Of December 14, 2012 On Waste (I.E., Journal Of Laws Of 2023, Item 1587, As Amended), 2) The Act Of November 17, 2021 Amending The Act On Waste And Some Other Acts (Journal Of Laws Of 2021 Item 2151, As Amended), 3) Of The Regulation Of The Minister Of Health Of October 5, 2017 On A Detailed Manner Of Dealing With Medical Waste (Journal Of Laws Of 2017, Item 1975), 4) Of The Act Of 27 April 2001 Environmental Protection Law (I.E., Journal Of Laws Of 2024, Item 54, As Amended), 5) Of The Act Of 19 August 2011 On The Transport Of Dangerous Goods (I.E., Journal Of Laws Of 2024, Item 643) And Agreements Regarding The International Road Transport Of Dangerous Goods - Adr, 6) Of The Act Of 6 September 2001 On Road Transport (I.E., Journal Of Laws Of 2024, Item 1539, As Amended). 3. Pursuant To Art. 95 Paragraph 1 Of The Act, The Employer Requires That All Activities Be Carried Out By Persons Employed On The Basis Of An Employment Contract. This Requirement Therefore Applies To People Who Perform Activities During The Implementation Of The Order: Loading, Unloading, Disposal Of Medical Waste. 4. Requirements Regarding The Method Of Documenting Employment Of Persons Under An Employment Contract, Control Rights Of The Contracting Authority And Sanctions For The Lack Of Employment Of Persons Under The Employment Contract Have Been Specified In Detail In The Contract Template (Annex 9 To Swz). 5. The Purchaser Does Not Reserve The Possibility Of Applying For The Award Of The Contract Only By The Contractors Referred To In Art. 94 Of The Act.
