4.1. The Subject Of The Contract Is Providing Assistant Social Services For The Participants Of The Project 65 Years And More As Part Of Task No. 2: Creating Conditions And Services In The Field Of Care For People In Need Of Support In Everyday Functioning At The Place Of Residence For Residents Of The Krasnobród Commune In The Project Senior Policy Efs+ In The Years 2024-2025 As Part Of The European Funds Program For Lublin 2021-2027 Co-Financed From European Funds Social Fund+, Implemented By The Regional Social Policy Center In Lublin In A Partnership With 36 Municipalities From The Lublin Province, Including The Krasnobród Commune On The Basis Of A Partnership Agreement Of 27 October 2023.4.2. The Scope Of The Subject Of The Contract Includes In Particular: The Provision Of Assistant Social Services In The Krasnobród Commune In The Place Of Residence Of The Project Participant, Esf+Senior Policy, Whose Material Scope Includes The Provision Of Assistant Social Services In The Krasnobród Commune For Min. 60 Project Participants. The Number Of Hours Per Project Participant Is - About 90 Hours Of Service Throughout The Duration Of The Contract (Adapted To The Individual Needs Of The Project Participant). The Total Number Of Hours Of Services Under The Order Is 5,400 Hours. The Detailed Scope Of Service Provision Was Specified In Point 4.3 Swz.4.3. The Scope Of Assistant Services Adapted To The Individual Needs Of The Participant In The Place Of Residence Includes In Particular: A) Support In Self -Service Activities, Including Maintenance Of Personal Hygiene And Care Activities (E.G. Washing The Head, Body Washing, Bath, Combing, Shaving, Cutting Hands Of The Hands And Legs, Change Of Position, E.G. Switching From Bed/Chair To A Trolley, Arranging In Bed, Preventing Pressure Ulcers Or Burns, Preparing And Eating Meals And Drinks (Including Peg And Probe), Bedding And Changing Bedding, Changing Diaper Pants And Hygiene Contributions; B) Support In Running A Household And Filling In Social Roles (In The Case Of Independent Residence (E.G. Cleaning The Apartment, Including Everyday And Sanitary Facilities, And Launching Garbage, Making Ongoing Purchases (Accompanying A Disabled Person In A Store - E.G. Informing Him About The Location Of Goods On Shelves, Serving Goods From Shelves, Putting Goods In A Basket/Shopping Trolley, Boring A Basket (Up To 5 Kg), Running A Disabled Person Or A Trolley Shop Wheelchair, Help With The Ticket Office), Window Washing Up To 2 Times During The Provision Of Services, Keeping Clean And The Efficiency Of Equipment Facilitating Daily Functioning (E.G. Trolley, Balcony, Lift, Balls, Electric Toothbrush, Electric Shaver, Etc.), Washing And Ironing Clothing And Bedding, Possibly Their Handing Over And Pickup From The Laundry (In The Presence Of A Disabled Person), A Detailed Description Of The Subject Of The Contract Was Specified In Chapter 4 Swz, I.E. The Description Of The Subject Of The Contract.
