The Subject Of The Contract Is The Provision Of Medical Services Covering The Performance Of Protective Vaccinations For Soldiers Of Military Units And Institutions On The Supply Of 34 Military Economic Department: 2. The Subject Of The Contract Covers: • Qualification Examination Of A Doctor In Order To Exclude Contraindications To Perform A Protective Vaccination; • Exercise Vaccination With A Vaccine Provided By The Contracting Authority; • Documenting The Protective Vaccinations Performed And Reporting The Medical Services Rendered By Sending Completed Documents In Electronic Form To Cre Sz Rp Through Earlier Downloading An Application That Has Been Available For Download On The Cre Szp Website In The Files To Download Folder (Www. Cresz.Wp.Mil.Pl) Or Sending In Paper Form Epidemiological Of The Polish Armed Forces, Ul. Col. K Leski 7, 01-485 Warsaw. 3. The Subject Of The Contract Was Divided Into 4 Parts: 1) Part 1: Protective Vaccinations In Rzeszów, 4. The Contractor Undertakes To Perform Services In Accordance With The Act Of 5 December 2008 On Preventing And Combating Infections And Infectious Diseases In Humans By Authorized Medical And Nursing Staff Entitled To Perform Services, On Business Days, I.E. From Monday To Friday From Friday From 7.30 To 15: 00.5. The Contractor Is Obliged To Instill All The Persons Who Have Applied For Vaccination Of The Day. 6. The Employer Requires For Reporting To Send The Number Of Vaccinated People To The Last Month. 7. The Contracting Authority Reserves That The Delivery Of Vaccines Will Take Place During Working Hours, I.E. From Monday To Friday From 9:00 To 14:00 (Excluding Public Holidays) After Prior Telephone Determination. The Day Of Delivery Is Considered The Day On Which Both Parties Will Be Signed By The Delivery And Acceptance Protocol, The Template Of Which Will Be Determined After Signing The Contract. Vaccines Will Be Delivered In Factory New, Undamaged Packaging Marked In Accordance With The Regulations In Force In Poland, In Conditions Ensuring Usefulness And Guaranteeing Proper Protection. The Cost Of Transporting Vaccines Is Covered By The Contracting Authority. 8. A Detailed Description Of The Subject Of The Contract Is Annex 4 To Swz. An Integral Part Of The Description Of The Subject Of The Contract Are: The Forms Of The Offer Price Calculation (Annexes 1A, Annex 1B, Annex 1C, Annex 1D To Swz) And The Provisions Of The Draft Contract (Annex 5 To Swz).
