The Subject Of The Contract Includes Successive Delivery Of Cytostatics Along With Transport To The Headquarters Of The Ordering Party - Municipal Hospital No. 4 In Gliwice Sp. Z O.O. - Hospital Pharmacy, Ul. Zygmunt Stary 20, 44-100 Gliwice And Unloading To Pharmacy Warehouses At The Contractors Expense. In The Event Of Concluding The Contract, Invoicing Will Take Place On One Invoice, In Accordance With The Contract For All Parts Won By A Given Contractor. The Employer Does Not Allow The Possibility Of Issuing Collective Invoices Covering Delivery Covered By Other Contracts Concluded Between The Employer And The Contractor. The Contracting Authority Requires That The Contract Number Should Be Included On The Invoice. Due To The Software Used By The Employer, Any Interference In The Assortment And Price Form Is Prohibited. Forms Should Be Sent In An Unchanged Format .Xls, .Xlsx Or Other Spreadsheet Format. Changes Are Not Allowed By Adding Columns, Removing Columns, Removing Sheets Of Individual Parts And Others. The Contracting Authority Expects The Unit Price For 1 Item To 2 Decimal Places. The Packaging Should Be Calculated With Rounding Up To Full Packaging. In The Event Of A Change In The Number Of Packaging, The Contracting Authority Requires That The Contractor Change The Color Of The Modified Line To Red.
