1. The Subject Of The Order Is The Construction Of Ul. Świerkowa In Otrębusy On The Section From Ul. Malinowa To The Entrance To The Mazowsze Conference Center Building.2. The Material Scope Of The Subject Of The Order Includes The Performance Of All Works In Accordance With The Documentation Constituting Annex 5 To Swz.3. On The Date Of Signing The Contract, The Contractor Will Prepare Simplified Cost Estimates For The Road, Drainage, Street Lighting, Power, Technological Channel, Fiber Optic Network And Gas Connections Sectors, For The Total Amount Specified In § 3 Section 2 Of The Contract, Constituting Annex No. 2 To The Contract.4. The Contractor Provides A 60-Month Warranty For The Works Performed Against Defects, Counting From The Date Of Signing The Final Acceptance Report Of The Works.5. Pursuant To Art. 95 Of The Public Procurement Law, The Ordering Party Determines The Employment Requirements For The Contractor Based On The Employment Relationship Of Persons Performing Activities Involving Paving Works.5.1. The Contractor, Before Commencing The Implementation Of The Subject Of The Contract And No Later Than 5 Days From The Conclusion Of The Contract, Is Obliged To Submit To The Ordering Party: A) The Contractors Declaration Of Employment On The Basis Of An Employment Relationship Of Persons Performing The Activities Referred To In Section 5,B) Copies Of Employment Contracts Of Persons Performing The Activities Referred To Above During The Execution Of The Order, Certified As True Copies By The Contractor, Respectively. The Contractors Declaration (Together With A Document Regulating The Scope Of Duties, If Prepared), Or Other Documents.5.2. Documents Constituting Evidence Of Employment Based On The Above-Mentioned Employment Relationship. Persons May Be Presented In The Form Of Anonymized Copies, In A Way That Ensures The Protection Of Employees Personal Data, In Accordance With The Provisions On The Protection Of Personal Data (I.E.
