The Subject Of The Order Is The Provision Of The Service Of Confirming Referrals For Spa Treatment Or Spa Rehabilitation, In Accordance With The Principles Set Out In The Swz, Including The Draft Provisions Of The Contract. Confirmation Of Referrals For Spa Treatment Or Spa Rehabilitation Will Be Made In Paper Form (For Paper Referrals) Or Electronically ( For E-Referrals). In The Case Of Electronic Referrals (E-Referrals), The Contractor Undertakes To Perform The Activities Covered By The Order In Electronic Form In The Application Of The Spa Treatment Service System (Solu). The Order Can Only Be Performed On Computer Equipment Provided By The Silesian Ow Nfz, Which Can Only Be Used For The Purpose Order Fulfillment. The Ordering Party Will Provide The Contractor With Access To A Vpn Account And Access To The Application Of The Spa Treatment Service System (Solu). The Service Being The Subject Of This Order Should Be Performed In Accordance With Applicable Law, Including The Regulation Of The Minister Of Health Of July 7, 2011 Regarding Referral To Spa Treatment Or Spa Rehabilitation (Consolidated Text: Journal Of Laws Of 2024, Item 208) And In Accordance With The Rules Medical Knowledge. The Service Covered By The Subject Of The Order Consists In Performing The Following Activities - Scope Of Duties Of A Specialist Doctor: 1) Assessing Referrals For Spa Treatment Or Spa Rehabilitation In Terms Of Their Purposefulness (Approval Or Lack Of Approval) And Indicating The Profile And Type Of Spa Treatment Or Spa Rehabilitation, 2) If Necessary, Applying To The Health Insurance Doctor Issuing The Referral To Supplement Or Update The Medical Documentation Or To Conduct Additional Tests Necessary To Determine The Type And Scope Of Spa Treatment,3) Re-Evaluating Referrals After Supplementing Or Updating The Medical Documentation Made By Health Insurance Doctors; 4) Justifying The Qualification Of The Referral In The Event Of Contraindications Or Lack Of Indications For Spa Treatment Or Spa Rehabilitation; 5) Assessing In Medical Terms The Appeals Of The Insured Regarding The Confirmation Or Non-Confirmation Of Referrals For Spa Treatment Or Spa Rehabilitation Or The Selection Of A Given Date/Direction/Place Of Treatment;6) Issuing Medical Opinions On Returns Referred For Treatment Spa Treatment Or Spa Rehabilitation;7) Issuing Medical Opinions On Complaints Of Insured Persons Or Childrens Guardians;8) Issuing Medical Opinions On The Qualification Of Spa Treatment Or Spa Rehabilitation Questioned By Beneficiaries;9) Issuing Other Written Medical Opinions As Part Of Handling Matters Related To Treatment Spa Treatment Or Spa Rehabilitation. The Order Covers A Maximum Number Of 199,405 Referrals. The Ordering Party Guarantees The Execution Of The Order On A Minimum Level Of 1,000 Referrals Per Month Submitted To The Contractor For Processing Under The Contract. The Order Will Be Executed Under The Terms And Conditions Specified In The Draft Provisions Of The
