The Subject Of The Order Is The Provision Of Medical Services In The Field Of Occupational Medicine, Including For Sanitary And Epidemiological Purposes And Authorization To Drive Vehicles, On Average Annually For A Group Of 1,550 Employees Of The Medical University Of Warsaw And Candidates For Employees, I.E.: 1) Academic Teachers Employed In Research Positions , Research And Teaching And Teaching Staff As Well As Scientific And Technical And Engineering And Technical Staff Who Conduct Or Support Teaching Classes And Carry Out Work Scientific And Research In The Disciplines Of Medicine, Dentistry, In Connection With The Provision Of Health Services In Clinical Hospitals, Including Those Employed In Places Where Health Services Are Provided And In Places Where Scientific And Research Work Is Carried Out,2) Academic Teachers Employed In Research, Research And Teaching Positions And Teaching And Scientific-Technical And Engineering-Technical Employees Who Conduct Teaching Classes And Carry Out Scientific And Research Work In The Field Of Pharmaceutical Sciences, The Field Of Health Sciences, The Field Medical Sciences And Biological Sciences, Including Those Employed In Places Where Health Services Are Provided And In Places Where Scientific And Research Work Is Carried Out,3) Employees In The Administration, Library And Scientific Information Groups, Service Workers And Drivers,4) Residents Implementing Specialization Programs In Units Wum, - Hereinafter Referred To As Medical Examinations, Specified In Terms Of Type And Number In The Offer Form, Constituting Appendix No. 1 To The Swz, Under The Conditions Specified In The Draft Agreement, Constituting Annex No. 2 To Swz. Medical Services Constituting The Subject Of The Order, Excluding Tests For The Purpose Of Obtaining A Driving License, I.E. All Tests And Specialist Consultations As Well As Collecting Material And Performing Diagnostic Tests Necessary To Issue A Medical Certificate, Will Be Carried Out Between: From 8:00 A.M. To 6:00 P.M. On Working Days From Monday To Friday, Excluding Public Holidays, At The Contractors Facility, Located At A Distance Enabling And Wigury, Fr. Trojdena And Pawińskiego, Accessible On Foot Or By One Public Transport In No More Than 15 Minutes. Tests For Admission To Driving Will Be Performed At A Facility Located In The City Of Warsaw.
