Tenders Are Invited For Footwear And Operation Theater Footwear

Tenders are invited for Supply of Footwear and Operation Theater Footwear. Applications are invited for the registration of suppliers, manufacturers, Government Institutions including registered Co-operative Societies, reputed commercial establishments and individuals for the supply of following goods and services for year 2025. All applications may either be sent by registered post addressed to Director. Castle Street Hospital for Women, Colombo 08 or hand delivered. The caption, Registration of Suppliers for year 2025 should be mentioned on the left-hand top corner of the envelope containing the application. It is mandatory to submit the Business Registration Certificate with the application and the applications without such certificates will be rejected. Instructions and Conditions 1. A non-refundable registration fee of Rs. 1,000.00 should be paid in cash for each item and service to the Shroff of the Castle Street Hospital for Women and the Original of the Receipt (Form General 172) should be attached to the application. (Cheques and Money Orders will not be accepted). Suppliers may register for one or more items and registration fee should be paid for each item separately. If sending more than one application, all applications may be sent under one envelope. Application forms may be obtained from the office between 9.00 a.m. and 3.00 p.m. on weekdays from 25.11.2024 until 13.12.2024. 2. Suppliers should agree to supply goods and sendees on credit basis at least for one month and the transport should be provided by the supplier. 3. All applications may either be sent by registered post addressed to Director, Castle Street Hospital for Women, Colombo 08 or deposited in the Tender Box kept at the Office of the Accountant to be received before 10.00 a.m. on 16.12.2024. Late applications will be rejected. 4. The caption, Registration of Suppliers - 2025 should be mentioned on the left-hand top corner of the envelope containing the application. 5. The Director of the Castle Street Hospital for Women, Colombo OS reserves the right to accept / reject any application received for registration without showing any cause.
