The Subject Of The Order Is The Successive Delivery Of An Aqueous Solution Of Polyaluminum Chloride, In The Amount Of 360 Tons (Hereinafter Also Referred To As The Product) - Used To Treat Water Intended For Human Consumption, Transported By The Contractor To The Kozłowa Góra Water Treatment Plant, Ul. Leśna 34, 42-583 Wymysłów. 2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 5 To The Swz. 3. Detailed Conditions For The Execution Of The Order Are Included In Annex No. 6 To The Swz. 4. The Means Of Proof In Question: 4.1. In Order To Demonstrate That The Offered Product Complies With The Requirements Specified In Annex No. 5 To The Swz - Opz, The Ordering Party Requests That The Contractor Submit The Following Evidence Together With The Offer: 1) A Current Hygienic Certificate Issued By The National Institute Of Public Health - National Institute Of Hygiene, Allowing The Product To Be Used In Water Intended For Human Consumption; 2) Technical Specification Of The Offered Product. Note: The Technical Specification Should Confirm Compliance With All Requirements Specified In Point 2 Of Annex 5 To The Swz - I.E. In The Description Of The Subject Of The Order. The Ordering Party Will Accept Equivalent Means Of Evidence If They Confirm That The Offered Deliveries Meet The Requirements Specified By The Ordering Party. 4.2.
