1. The Subject Of The Order Is The Provision Of Specialized Care Services For Adults, Children And Adolescents With Mental Disorders In Their Place Of Residence. The Contractor Undertakes A Task In The Field Of Social Assistance, In Accordance With The Provisions Of The Act Of 12 June 2004 On Social Assistance (Journal Of Laws Of 2024, Item 1283, As Amended), Consisting In The Implementation Of Specialized Care Services For Adults, Children And Adolescents With Mental Disorders In Their Place Of Residence In The Grudziądz Commune, To Whom The Ordering Party Granted These Services By An Appropriate Administrative Decision. Specialized Care Services Include Activities Specified In The Provisions Of The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2024, Item 816).2. The Subject Of The Order Includes The Provision Of Specialized Care Services For Adults, Children And Adolescents With Mental Disorders In Their Place Of Residence, Consisting Of: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating Activity, Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Supporting, Also In The Form Of Assisting In Everyday Life Activities; 2) Care - As Support For The Treatment Process, Including Assistance In Access To Health Services, Agreeing And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, Helping In Purchasing Or Ordering Medicines At A Pharmacy, Supervising The Intake Of Medicines And Observing Possible Side Effects Of Their Use, Helping In Reaching Health Care Facilities And Rehabilitation Facilities;3) Physical Rehabilitation And Fitness In Various Forms Therapy Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004. On Health Care Services Financed From Public Funds (Journal Of Laws Of 2024, Item 146, As Amended).3. An Estimated 6 Children And 2 Adults Will Benefit From Specialized Care Services For Approximately 224 Hours Per Month, I.E. 2,688 Hours During The Duration Of The Contract.
